Community College Transfer Honor Scholarship
Application
The San Francisco State Community College Transfer Honor Scholarship is available to incoming
transfer Alpha Gamma Sigma or Phi Theta Kappa or Community College campus Honors program
students on a competitive basis in recognition of academic excellence and financial need.

Eligibility Requirements
1. Be an Alpha Gamma Sigma or Phi Theta Kappa Honor Society member or a member of the
Community College campus Honors Program at a California Community College.
2. Entering Junior from a California Community College.
3. Minimum 3.0 cumulative GPA.
4. Have financial need as determined by the SF State Office of Student Financial Aid. Apply or reapply
online using the 2014-2015 Free Application for Federal Student Aid at www.fafsa.ed.gov OR
students who meet the AB540 definition, apply using the 2014-2015 CA Dream Act Application at
www.caldreamact.org (apply by the March 2, 2014 priority deadline).
5. Enroll full-time (12 units) each semester at SF State for the 2014-2015 academic year.
6. A recommendation letter on letterhead from an Alpha Gamma Sigma advisor or Phi Theta Kappa
advisor or the Community College Honors Program director/advisor or from a faculty member at your
current college. If the recommendation letter is from a faculty member, you must also submit a letter
from your honor society or honors program advisor to confirm your membership.
7. Write a personal statement (see scholarship application for guidelines).
8. Submit or mail: the completed and signed scholarship application, signed letter of
recommendation on letterhead & confirmation of membership, an unofficial academic
transcript from all Community Colleges and other colleges attended and your personal
statement to the SF State Office of Student Financial Aid by Monday, March 10, 2014.
Procedures
Completed applications that meet all of the above criteria and are received or postmarked by the March 10th
deadline will be reviewed by the scholarship committee. Students selected to receive a scholarship will be notified
by May 2014. Students not selected will be notified shortly thereafter.

Write a personal statement in which you tell the scholarship committee about yourself.
Address the following questions in your personal statement and give specific examples.
Statement must be written in essay format, typed, double-spaced, 12 point with 1” margins and
must be at least 2 pages, but not to exceed three pages.
- How have you contributed to your community or college in the areas of diversity or leadership
or service?
- What are significant factors or events in your life that have impacted your desire or ability to
attend college?
- Who has inspired you?
- What academic achievements are you proud of?
- What are your educational and career goals?
Note: All information collected as a part of the application process for the Community College Transfer
Honor Scholarship will become the property of the Scholarship Committee, will be kept confidential, and
will only be used by the Scholarship Committee for purposes of evaluating applicants.
My signature below affirms that I have read, understand, and meet all the requirements of the Community
College Transfer Honor Scholarship and that the information presented in this application is accurate and
true to the best of my knowledge. I understand that if I am awarded this scholarship, I must enroll
full-time (12 units) each semester at SF State for the 2014-2015 academic year.
I understand that if I also receive Federal and/or State financial aid for the 2014-2015 academic year, the
scholarship will be coordinated with my financial aid.
I give permission to SF State to check University records to verify eligibility requirements for this
scholarship and to release my information to the donors of this scholarship.
