Colorectal Services at Carlton DTC Background: I have been an Endoscopist since 1994 and since 1996 have provided endoscopy services from my practice for my patients and those of other practices. The current facilities are up to date and specifically designed for endoscopy provision. I also work at BTHT as a Hospital Practitioner providing diagnostic & therapeutic Colonoscopy, Flexible Sigmoidoscopy and Gastroscopy. The service: The service I provide at Carlton is a one stop colorectal service which I started in 1996 because no such service existed locally. I will see, investigate and manage any colorectal condition including: Rectal bleeding Change in bowel habit Diarrhoea and constipation Perianal conditions including non-prolapsed piles (treated by sclerotherapy) and fissures Anaemia Abdominal pain IBS type presentations All patients will be investigated and managed within my service where possible. Most will have flexible sigmoidoscopy and proctoscopy as per guidelines agreed with the local gastroenterologists and colorectal surgeons. I have direct access to further investigations including Colonoscopy & Gastroscopy (including fast track slots), CT Colonoscopy, Barium Enema as well as Hydrogen breath testing and SeHCAT scans for diarrhoea. I also plan to organise direct booking onto surgical lists for perianal conditions. What do the patients think? – we get and have always got extremely high levels of patient satisfaction with our service. We perform biannual surveys. How do you refer? SystmOne Task (share to Carlton Medical Practice and send either an unassigned task or to me directly), Choose and Book or by Letter Cancers: Should any cancers be discovered I also have direct access to the fast track colorectal team. As I see all referrals within 2 weeks this service actually results in a faster track than referring into the fast track system. We have full tracking in place for all investigations or fast track referrals made from my service. However if you have a high suspicion of colorectal cancer please still refer via the fast track service. Governance – this service has been re-accredited many times by the PCT – the last re-accreditation was in 2011 for 3 years. GRS (Global Rating Scale) returns are made nationally from the unit biannually. We are planning to achieve JAG accreditation for our unit. How much? – this service is extremely competitive being approximately 1/3 of the National Tariff price for the equivalent service. Why are you telling us about this now? – I have recently made changes at my practice to increase my capacity hence I now have the ability to flex capacity and meet my target of a 2 week wait for all new referrals and thus can advertise the service more widely than previously.
