(a) Declaration Statements and Statements of Policy
Statement of Policy regarding fitness to practise proceedings by a licensing/regulatory body and
relating to criminal investigations in the UK or overseas.
Registration with the General Medical Council or General Dental Council imposes on doctors and
dentists the duty to provide a good standard of medical care for, and to behave appropriately,
towards patients. NHS Employers also have a duty to ensure that patients receive a good
standard of medical care and ensure as far as possible the safety of patients. We therefore need
to establish if you have been found guilty of a criminal offence, been bound over or cautioned or
are currently the subject of proceedings which might lead to a conviction, an order binding you
over or a caution, in the UK or any other country.
Applicants for posts in the NHS are exempt from the Rehabilitation of Offenders Act 1974.

JOB DESCRIPTION
SPECIALTY DOCTOR IN OLD AGE PSYCHIATRY (10 PA’s)
1. Introduction
As part of the Angus Older People's Services strategy and priorities, expansion of specialist Old
Age Psychiatry medical input has been agreed. The successful partnership working with Medicine
for the Elderly, Primary Care and Local Authority has resulted in an appropriate increase of
referrals to our service which thrives to improve standards of care for older people with mental
disorders in Angus.
A full time post as Specialty Doctor in Old Age Psychiatry has been created to achieve the above
by mostly supporting the 2 Consultants Old Age Psychiatrists in the South Locality of Angus
(Arbroath, Carnoustie, Monifieth).
2. Details of the Post

Principle Duties:
The person appointed to this post will provide, in conjunction with other medical staff, the
provision of specialist psychiatry of old age service to the population of South Angus and input to
the Angus-wide functional in-patient unit based at Stracathro Hospital. The appointee will assist in
the assessment of new patients, carry out treatment and follow up under consultant supervision in
a variety of different settings. The main areas of responsibility will be:

Community work in the South Locality will include working with the current Old Age
Psychiatry Consultants Drs Ashleigh Duthie (covering Arbroath and Monifieth) and Cesar
Rodriguez (covering Carnoustie). Out-patient clinics, domiciliary visits and visits to care
homes will be part of the community work aspect of this post. The postholder will be a
member of the Community Mental Health Team for Older People (CMHT-OP) for the
South Locality based at Gowanlea, Arbroath. This is an integrated Health and Social Care
CMHT.

The Specialty Doctor will work closely with other Angus-wide community services such as
the Angus Dementia Liaison Team (for care homes and community hospitals) and the
Angus Dementia Postdiagnostic Support Team for those patients and care homes within
the South Locality.

Inchape Unit at Arbroath Infirmary, is a 15-bedded Specialist Dementia Unit serving the
population of South Angus. Dr Ashleigh Duthie is the Consultant leading this team and
working with her in this unit might be part of the duties within the post.

Liaison work might be undertaken at Arbroath Infirmary where two Medicine for the Elderly
wards are located under Dr Dougie Lowdon, MfE Consultant.

The Specialty Doctor will work up to 2 sessions a week within Rowan Unit, our Angus-wide
functional in-patient unit located at Susan Carnegie Centre (modern, purpose-built centre
that opened in December 2011), Stracathro Hospital. The multidisciplinary team based at
Rowan Unit in led by Dr David Rooke, Consultant in Old Age Psychiatry with whom the
Specialty Doctor will work closely.


This role does not include any out of hours on call.
4. Angus Psychiatry of Old Age
Catchment Areas:
The Old Age Psychiatric Service in Angus is sectorised and each of the three areas is served by
a Community Mental Health team and is coterminous with the Local Authority boundaries. The
successful candidate will cover the South Angus area which includes the small towns of
Arbroath, Carnoustie and Monifieth and their surrounding areas with an over 65 population of
approximately 10.000 (around 45% of the elderly population in Angus).
Patients under the age of 65 with dementia are generally looked after by the Old Age Psychiatry
Service , but may have access to parts of the under 65 service if this is clinically appropriate.
Although it has been identified as a development priority area, there are currently no specialised
facilities or resources within Tayside for this group (although Dr David Rooke together with
colleagues from Neurology and Neuropsychology run a diagnostic clinic in Ninewells Hospital for
young onset cognitive impairment patients).

Angus Older People Psychological Therapies Service:
There are currently two F/T Clinical Psychologists working for Older People with mental health
problems in Angus.
Dr Fiona Macleod Consultant Clinical Psychologist has clinical commitments in the South Angus
locality. She is also the Lead Clinician for the Tayside-wide Older People Psychological
Therapies Service.
Dr Paula Mulholland, Clinical Psychologist, provides psychological assessment and treatment to
people in the North East and North West Angus catchment areas.
The Angus Older People Psychology Department regularly has Psychology Trainees rotating for
6 months blocks during their postgraduate training and it is actively involved in staff training, audit
and research.
Community Mental Health Teams for Older People
Each of the sectors has a Community Mental Health Team (CMHT) for those with dementia and
those over 65 with a mental illness. These well established Teams were the first ones in Scotland
to be integrated teams between Health and Social Work. Therefore, team workers from NHS and
Social Work can organise care packages, respite or admissions to care homes if necessary.
In South Angus, the CMHT is Gowanlea, Arbroath and referrals (usually from GPs and other
social work teams) are allocated at the weekly team meeting.

In-patient Facilities
There are two wards located in Susan Carnegie Centre, Stracathro Hospital and two Specialist
Dementia Wards located in Forfar and Arbroath.
Functional Assessment Ward – Rowan Unit
Rowan Unit, Susan Carnegie Centre, is a 12 bedded mixed gender ward that caters for those
over 65 from Angus with a functional illness who require in-patient assessment and treatment.
There is a well-established multidisciplinary team and Dr David Rooke provides consultant
responsibility to all patients admitted to this ward.
Specialist Dementia Ward – Willow Unit
Willow Unit, Susan Carnegie Centre, is a 15 bedded mixed gender ward for the assessment and
treatment of people with dementia mainly from the North East Locality but also from the rest of
Angus whose presentation warrants their admission here rather than in their own locality unit. Dr
David Rooke has consultant responsibility for patients admitted to this ward.
Specialist Dementia Ward – Prosen Unit
Prosen, located in Whitehills Hospital, Forfar, is a 15 bedded purpose-built unit for people with
dementia and challenging behaviour that require in-patient assessment and treatment as well as
intermediate care. Dr Susan Logie is the consultant with responsibility for this unit.
Specialist Dementia Ward – Inchcape Unit
Located in Arbroath Infirmary, this ward is for people with dementia and that require in-patient
assessment and treatment as well as intermediate care. Dr Ashleigh Duthie is the consultant with
responsibility for this unit.
Munro Day Services
This is NHS day service for older people in Angus with a functional psychiatric illness. It is located
in Susan Carnegie Centre, Stracathro.
Staff carry out assessment and treatment both in the unit and also in patients’ homes throughout
Angus. Individualised care plans are designed for each patient and they might receive one to one
in-put or join specific groups. Group work takes place in the unit but also in different locations in
Angus so patients can attend more easily in their own community. Examples of groups include:
coping with depression, relaxation, anxiety management, self esteem, lifestyle.

Dementia Liaison Team
There is no traditional NHS Day Hospital provision for people with dementia in Angus. Instead,
moneys have been secured to set up a dementia liaison team.

The work carried out by the Dementia Liaison Team predominantly includes supporting Care
Homes and Community Hospitals looking after people with dementia that present with BPSD,
facilitate the transition from POA in-patient units to Care Homes, review of antipsychotic
medication in Care Homes, teaching and training of staff in such facilities.
5. Other Services for Older People in Angus
Angus Medicine for the Elderly Service
Older People Services within Angus CHP includes not only Psychiatry of Old Age but also
Medicine for the Elderly (MFE); both specialties are therefore under the same management
structure.
We enjoy a very good and close relation with our colleagues from MFE who provide liaison visits
to our in-patients at Susan Carnegie Centre and as required for our in-patients in the two
Specialist Dementia Care Units located in Forfar and Arbroath.
MFE Service has three main bases in Angus: in Stracathro with an Assessment and
Rehabilitation ward (14 beds), a Stroke Unit (10 beds) and Day Hospital provision, in Arbroath
Infirmary with acute MfE wards (20 beds) and in Whitehills Hospital with an acute MfE ward, Isla
Ward (16 beds). They also have slow stream rehabilitation beds in Arbroath (12 beds). They run
clinics in different sites in Angus for Parkinson’s Disease, Falls and TIAs as well as generic
geriatric clinics.
MFE medical staff include:
- Dr Ian Gillanders, Consultant. F/T. Based at Stracathro, North East.
- Dr Douglas Lowdon, Consultant. F/T in South Angus.
- Dr James Shaw, Consultant, F/T in Forfar, North West Locality.
- Dr Greg Cox, Associate Specialist. F/T. Based Stracathro/Arbroath.
- Dr Alf Kumar, Specialty Doctor. P/T (6 sessions). Based at Stracathro.
- Dr Gordon Stewart, Specialty doctor P/T (8 sessions) in Forfar.
Dr Gillanders organises the post-graduate programe for MFE in Stracathro which includes
monthly meetings with Dr John Tainsh, Consultant Radiologist, at Stracathro when neuroimaging
cases are discussed.
Dementia Postdiagnostic Service
In early 2005, Joyce Murray was appointed Angus Early Stage Dementia Coordinator. This was a
joint project between Health and Social Work created to help people newly diagnosed of
dementia and their families in Angus. It has become an extremely valuable part of the Service
and the moneys for it were secured in 2007. A second worker, John Gibb, was appointed in 2011.
They are now the Angus Dementia Postdiagnostic Service.
Patients newly diagnosed are referred to Joyce or John who initiate a series of contacts with them
and their families to provide information and support. Among other issues, she discusses the
diagnosis, legal matters, driving, welfare benefits, further information and support available.
In collaboration with Alzheimer Scotland, they run a group for younger people with dementia in
Angus (“The Voyagers Group”) and a support group for their partners (“The Wasps”), as well as
men's groups.
This Service has been recognised as a good example of partnership working and service delivery
for people with dementia and their families. A DVD explaining the Service has been produced and
it can help people with dementia and their families to become familiar with the Service.

6. Teaching, Clinical Governance and Continuing Education
There are good links with the Department of Psychiatry, Dundee University and the postholder
will also be encouraged to participate in the teaching and training of other disciplines, including
nursing staff, AHPs, Mental Health Officers, etc.
Research is encouraged and supported. It is expected that the appointee will play a part in
Clinical Governance including Clinical Audit activities; both locally based collaborative audit and
multi-professional audit in a wider context.
There are a number of opportunities to continue medical education. There are ongoing
Postgraduate Medical Education Meetings, which have a variety of internal and outside speakers.
CPD sessions occur on a regular basis with consultant colleagues and other non-consultant
career grade staff. There is also a very active psychiatric postgraduate teaching in Angus every
Friday from 11.30, organised by Dr Chris Pell, Consultant GAP and College Tutor.

7. Terms and conditions of service
The appointment is subject to the Terms and Conditions of Service of Hospital Medical and
Dental Staff as applicable to Specialty Doctor Grade and the National Health Service (Scotland)
Superannuation Regulations.
The salary scales for the post are appropriate to the Specialty Doctor Grade in the NHS.
Specialty Doctor Pay Scale 2013-2014 is £37,176 to £69, 325 per annum.
The appointment will be on a 10 programmed activity per week basis. However, applications are
also welcomed from candidates who for personal reasons may be interested in working part-time.
