Goal #2: Provide continuity across programs for children and families Indicator #2: 75% of KY River programs implements Building Strong Foundation for School Success – Standards, Field Guide and Parent Guides in 2009 Goal #3: Provide supports to providers regarding child find process and Mental Health awareness Indicator 3a: Numbers increase in Leslie County for First Steps 0-2 from 2007 data Indicator 3b: Numbers increase for all KY River Counties in ECMH program with support for KY SEED Indicator 3c: The gap in services for children in the KY River region for children 2-4 will have identified services in regard to HANDS and Protection and Permanency needs Goal #4: Assess the revised community transition process Indicator: 100% of agencies will complete a survey of the process implementation provided by KECTP through Survey Monkey Steps/activities to support transition Lead Team Member and others who need to be involved 1a.
