Page 1 NHS Lanarkshire – Information Pack POSITION: Consultant Radiologist THE DEPARTMENTS The Radiology Directorate is part of the Cancer and Diagnostics Division. There is a radiology department in each of the three acute hospitals. The radiology directorate has a good record of innovation, both in terms of working practices and adoption of new technology. There is active encouragement of opportunities for role development of staff, where this will provide an enhanced service to patients. Lanarkshire were early introducers of IV injections by radiographers and radiographer barium enemas. Ultrasonographers on each site perform and report their own studies. Three radiographers are trained in A&E reporting and a further two are trained mammogram readers, supporting double-reporting of all symptomatic mammograms. Flexible working is encouraged, with, for example, several radiologists undertaking some of their radiology reporting sessions in the evening. The presence of a single Lanarkshire-wide PACS/RIS also allows radiologists to undertake some reporting at a different site from their base hospital, if convenient. Lanarkshire radiologists have utilised teleradiology to support CT reporting from home while on call since the late 1990s, and this is now provided by means of secure broadband links. Lanarkshire has had a full county-wide PACS system since 2001 and during 2008-09 was transferred to the national Carestream PACS. Voice recognition typing was introduced in 2009. Dr Fiona Gardner is the Associate Medical Director for the Division. Each hospital has a Site Clinical Director for Radiology. The Imaging Services Manager, Mr Eddie McGoldrick, has a deputy on each site. Page 2 NHS Lanarkshire – Information Pack POSITION: Consultant Radiologist THE DEPARTMENTS The departments are accredited by The Royal College of Radiologists for professional training and are affiliated to the West of Scotland Radiology Training Scheme. The radiology departments are formally associated with Glasgow Caledonian University for the practical training of B.Sc. Radiography students. The hospitals are recognised for post-graduate medical training in many specialties and for undergraduate training in Medicine, Surgery and Paediatrics from the Universities of Glasgow, Dundee and Edinburgh. Regular cancer multi-disciplinary team meetings are held with appropriate specialties, and there are less formal film review meetings with physicians, surgeons and care of the elderly specialists. A wide variety of radiology staff are involved in multi-disciplinary audit. The departments carry a wide range of bench books for day-to-day use. There are large seminar rooms (Wishaw and Hairmyres) with excellent facilities for viewing PACS images in clinical review meetings and tutorials. A video-conferencing link connects the Lanarkshire hospitals and the Beatson Oncology Centre, facilitating multi-disciplinary team meetings. IMAGING FACILITIES The departments provide a wide range of diagnostic and interventional techniques. They comprise diagnostic rooms including digital fluoroscopy, angiography/interventional, mammography, ultrasound, multi-slice CT and 1.5T MRI on each site. A fourth Lanarkshire CT scanner was installed in Hairmyres in 2009, with a fifth installed in Wishaw in 2013, in response to rising demand. This additional capacity is shared with Monklands Hospital, so that all three departments have equal access. RADIOLOGY MANAGEMENT SYSTEM Carestream RIS management system is in place, linked with the new Caresteam PACS. Page 4 NHS Lanarkshire – Information Pack POSITION: Consultant Radiologist POST INFORMATION The Post Applicants must possess the FRCR or equivalent higher professional qualification and be on the GMC specialist register for Clinical Radiology, or, for applicants currently in training, should have a CCT by the date of appointment. The post requires a comprehensive knowledge of all aspects of cross-sectional imaging. Development of a sub-specialty interest will be supported, and an interest in Breast imaging, ENT, MSK or Gastro-Intestinal imaging would be preferred. The post is full-time, offered at 10 PAs. There may be the option to increase this with EPAs, to a maximum of 12PAs in total. Consideration will also be given to applicants who, for personal reasons, may wish to work part-time. Some of the current post-holders work on a flexible basis, and the departments would be happy to discuss the possibilities for flexible working with applicants. Provision of cover for colleagues during annual, study or short-term sickness leave. Participation in the supervision, training and management of junior medical staff. Management responsibility where appropriate as agreed with the site clinical director. Participation in staff appraisal process. Participation in appropriate internal or external quality assurance programmes. Participation in clinical audit. Participation in both undergraduate and postgraduate teaching programmes. Compliance with Royal College of Radiologists’ recommendations on continuing Medical Education. Work on behalf of NHSL such as domiciliary consultations or services provided by other agencies, e.g. Scottish Prison Service. Radiology is a consultant-based service. Some functions have been delegated to other staff but the overall responsibility for all services remains with the consultants. The managerial head of each department is a consultant radiologist who has responsibility for the operational management of the department and its services Page 8 NHS Lanarkshire – Information Pack POSITION: Consultant Radiologist Clinical duties are rostered to achieve a balance between the special interests of individual consultants and the needs of the service. A rota details the day-to-day responsibilities of consultants. This will be as stable as possible and provides the basis for individual consultant job plans. Of necessity it is altered to take into account the absence of colleagues due to compensatory time off post on call, annual leave, study leave and short periods of sick leave (currently 2 weeks). The ‘core’ rota will include all direct clinical care and supporting professional activities of the consultant staff. Individual weekly rotas will also include any extra-programmed activities as contracted separately. Consultants will be available within the radiology department during all those programmed activities that involve direct clinical care. It is anticipated that most supporting professional activities will also be carried out within the hospital or related facility. Any SPA time spent elsewhere will be with the agreement of the Clinical Director. Working Hours - The routine working hours of the departments are Monday - Friday, 9am – 5pm, when a comprehensive in and out patient service is provided. Some radiologists choose, with the agreement of their site clinical director, to undertake some of their reporting sessions in the evening, rather than during the normal working day. In addition, all Lanarkshire acute hospitals provide a 7-day radiology service. At weekends and on public holidays, the designated on call radiologist undertakes routine in patient and A&E work during the daytime, as well as being available for emergency work out of hours. This allows all reporting to kept up to date, and optimises management of in patients, through 7-day availability of CT and ultrasound scanning. Daytime weekend working whilst on call is compensated by means of time off the following week. This arrangement has proved highly successful in providing routine 7-day access to imaging for the hospitals, and, due to the compensatory time off provided, is very popular with the radiologists. On-Call Rota - The on-call rota is shared equally by the Consultant Radiologists. A teleradiology link allows emergency CTs to be reported from home. There is an allowance in the job plan of 4 hours per week for unpredictable on call. Page 9 NHS Lanarkshire – Information Pack POSITION: Consultant Radiologist Work Programme As required under Section 3 of the Terms and Conditions of Service, the duties and responsibilities are supported by a job plan and work programme detailed as follows: Job Planning/Programmed Activities The job plan will be dependent on the successful candidate’s subspecialty training and interests. A minimum of 1 SPA will be included in the job plan. Additional SPAs (up to 2.5 in total) can be incorporated into the job plan depending on the time required to support the successful candidate’s professional activities and the needs of the service. On taking up post a Job Plan will be agreed between the person appointed and the Site Clinical Director /Associate Medical Director. The Job Plan will be reviewed informally after 3 months in post and then again on a formal basis after 6 months in post. Procedural DCC activities will include all aspects of cross-sectional imaging, and the work will represent a balance of general and specialist interest sessions. Work involving direct clinical care is composed of two types. First is procedural work (including CT, MRI, ultrasound, screening lists and routine reporting). Direct clinical care also includes ‘nonprocedural’, more flexible, clinical activity e.g. request card vetting, liaison with requesting clinicians, correspondence regarding patients and preparation for and attendance at multi-disciplinary clinical meetings. All DCC activities will initially be undertaken at the appointee’s base hospital. However, there may be a requirement in future to work in any of the NHSL acute hospitals. The detailed job plan will be agreed by the appointee and the Site Clinical Director, but all consultants are required to be flexible in relation to their agreed time-table in order to meet varying service requirements and cover for colleagues’ absences. The procedures set out in the ‘Terms & Conditions of Service’ must be followed if it is not possible to agree a job plan, either initially or at an annual review. Audit and research The successful candidate will be expected to participate in audit processes and there is an active clinical audit department available to assist. An interest in research will be supported. Continuing Professional Development Study leave is available within the terms and conditions of service with the approval of the Site Clinical Director/Associate Medical director. The appointee will be required to fulfill such demands for continuing professional development as the Royal College of Radiologists (or other relevant bodies) may make. Page 15 NHS Lanarkshire – Information Pack POSITION: Consultant Radiologist IMMIGRATION Overseas Doctors and Dentists Changes to Immigration Impact of Change Further Information For the purpose of immigration status, overseas doctors and dentists are those who, regardless of where they obtain their primary medical/dental qualification, are not nationals of the European Economic Area (EEA). A list of EEA countries is attached. On 7th March 2006 the Department of Health announced changes to the immigration rules for postgraduate doctors and dentists. The changes came into effect on 3rd April 2006 and mean that all doctors and dentists who wish to work in the UK from outside the EEA will need to meet the requirements of an employment category such as the work permit provisions. New applications for permit-free training visas will no longer be granted for doctors undertaking training posts Non-EEA doctors and dentists will still be able to come and train in the UK and are still eligible to apply, in open competition for training posts provided all the other eligibility criteria is met. However, training posts will now be considered employment for immigration purposes. Work permits will only be issued where there is a specific post for the overseas doctors that cannot be filled by someone from the EEA. The changes in immigration requirements for trainee doctors and dentists are a result of a Scottish Executive Health Department announcement and immigration issues are the remit of the Home Office.
