Minutes Triangle Breastfeeding Alliance (TBA) Meeting – February 6th, 2007 12 Present: Jam Gourley, Michele Hojnacki, Elaina Lee, Joan Levy, Bonnie Moore, Margie Mould, Vivien Omile, Mary Overfield, Teresa Roberts, Ann Skinner, June Wilensky and Diane Yelverton. We welcomed Teresa Roberts from Central Carolina Hospital of Sanford to her first meeting. More details on Megan Webb’s son: He was born 12/06/06 at 7 lb + 7 oz and 21” long. His name is Patrick Stone Webb and Caroline thinks he is great! (Breastfeeding is going well.) Corrections/additions to minutes from 1.02.07: None Treasury Report: TBA has $314.88 in the Credit Union, the TBA Scholarship fund (from magnet sales deposited in the Nursing Mothers of Raleigh treasury so people could be given tax deductible receipts) is $665.00. The profits from the 2006 conference ($996.58) will be deposited in our new bank account by Sheri. (Mary will serve as a back-up check signature for occasions if Sheri were unavailable.) Thank you to Bonnie (and June) for keeping TBA money safe in the Employees Credit Union until we could obtain a TBA bank account. Bonnie will make a listing of the sources of the deposits and the expenditures that have occurred to date $404.88 Credit Union balance $ 30.00 bank fees $374.88 $ 60.00 Capitol Vial check (has been replaced, but not deposited) $314.88 Current Credit Union balance Tax Status: We were told TBA could not get a fee-free checking account without official non-profit status, but discovered that is not the same as 501(c) 3 status. Mary contacted the IRS and we have been assigned the necessary Federal Employer Identification Number. 2007 Conference Expense Budget: We voted on Vivien’s idea and increased registration to $55 per person to include the cost of including a magnet in each handout packet. An exact budget would require input on honoraria/travel expenses for speakers and the number of free registrations for the planning committee. Ann will contact Dr. Dunn to find out what honorarium she would expect for speaking at the conference. Elaina will contact Heather Reeves to find out what honorarium she would expect for speaking at the conference. Mary will compile the attendance for us to determine which planning committee members get discounts on their registrations. TBA Display Update: Thanks to Jam for our new table top display board and case to be stored at Bonnie Moore’s office until the AWHONN conference in March. After that, it will be stored at Sheri’s home. Jam presented a draft of the panels she is creating for us so we could look them over before she laminates them. They looked fantastic! Bonnie has some eye-catching photos she took at our last conference for Jam to integrate into the display. AWHONN Conference: June and Bonnie agreed to be responsible for the TBA booth. They will also get chocolate to encourage people to stop by. Bonnie already has handouts about the TBA. Elaina got the magnet display to Mary to store until closer to March. Magnet Sales: Jam has featured the magnet/logo in the raised portion of the center panel of the display, so Bonnie and June are hopeful that some more magnets will be sold at the AWHONN conference. Family Practice Physician Education: Elaina has been asked to speak about the importance of breastfeeding for the Family Practice Program in Wilmington, NC, sometime in the spring. 1 Health Care Provider BF Support: TBA would like to promote a specific day during World Breastfeeding Week (WBW) as the day for health care providers to show their support of breastfeeding in whatever way they can. Wednesday, August 1st, 2007 was suggested. We would like to develop a flier with bulleted ideas to encourage planning among our health care professionals. Mary will look into our having a table display outside the cafeteria on August 1st and inquire about getting an article about the upcoming conference the WakeMed Microscope newsletter. Margie will inquire about having a statement on the August WakeMed paycheck stubs. UNC School of Public Health Intern: Diane has been granted an intern working on her masters in Public Health at UNC to work with TBA on the creation of the WBW BF Support Flier and possibly other projects. Elizabeth Roberts has a social marketing background and an interest in breastfeeding. Copies of her resume are available to those who were unable to attend this month. We all need to plan to meet for 1 ½ - 2 hours at our June meeting so the first hour can be devoted to going over the conference and still have time for us to meet as a group with Elizabeth to give her our ideas. Please bring your ideas and resources to that meeting to give her a good start. Several ideas were discussed: developing a pocket guide to put breastfeeding benefits and resources at healthcare provider’s fingertips, conducting surveys, etc. Planning for May 9th, 2007 – Fifth Annual Triangle Breastfeeding Alliance Conference Brochures: Thank you to Jam who created a draft of the brochure and will revise it to include our suggestions. Thank you to Diane who brought two photos for us to consider for the illustration on the brochure. We voted for an Asian mother kissing her baby. Jam and Vivien will find out how many brochures were mailed last year to private physician offices. Bonnie will check to see if WakeMed folders would be available for the handout packets. Vendors: The Andrews Center limit will be only 12 maximum for 2007. For profit exhibitors will have to pay a $60 fee (includes 1 lunch). Non profit exhibitors will only pay $10 for lunch. We need to offer a session to help people meet the certification requirement for IBCLE Ethics CERPS. All agree it needs to be a session with answers, not just “food for thought”. 2. Miriam Labbok (formerly of UNICEF) has agreed to speak on the implications of the WHO growth standards. 3. We would like to see her include stories of mothers who were so intimidated by the pressure to supplement to get their first babies to gain weight to meet the (as we now know) incorrect weight chart guidelines that they did not get to exclusively breastfeed not only their first babies, but also their subsequent infants. Ann Skinner is going to email Miriam case histories of mothers with whom she has worked for real-life examples in the presentation. 4. However, the growth standards will also make it very clear when a newborn is not feeding effectively and there are ethical considerations in those situations, too. 2 5. Implementing the new WHO Growth Standards will make us look at broader cultural ethical issues such as the need for a “Ban the Bags” campaign, the NC Blueprint, etc. Current issues: “Current Practice, Changing Issues” (also Poster sessions?) A “popcorn talk” with several speakers for short presentations on practice-changing issues: 1. “New” breast anatomy w/ ultrasounds showing no lactiferous sinuses: Elaina and Margie will create a handout with minimal Medela advertising. 2. Therapeutic breast ultrasound to treat plugged ducts: Elaina has confirmed Heather Reeves of Avanti Physical Therapy in Cary. 3. Reverse Pressure Softening (RPS) to manage engorgement: Jam and Michele can incorporate humorous visuals in teaching the technique. 4. Ann agreed to present case histories to discuss success by treating for nipple vasospasm when a mother experiences persistent sore nipples in spite of treatment for yeast and mastitis. Allergies and Breastfeeding Update: Confirmed Ann has located a speaker for an afternoon session: Dr. Karen Dunn at Carolina Pediatric Allergies. (Joan has also located a back-up speaker at Duke.) Ann will contact Dr. Dunn to arrange a time to meet with her to offer examples of the kinds of information our audience is seeking. We need suggestions for questions such as: should mothers avoid peanuts while pregnant? Cultural Panel: Rather than a panel of mothers, we will use knowledgeable health care professionals to address breastfeeding practices and appropriate support among different cultures. May Slowik, Pediatric Co-chief Resident, Duke University Medical Center will discuss breastfeeding and the Chinese culture. She was invited by Joan Levy Betsy Herman: WakeMed Raleigh lactation consultant who has lived in Ecuador will speak about cultural considerations when working with families from that country Lois Ugwuoke: WakeMed Raleigh Mother-Baby nurse who has worked as a midwife in Nigeria has agreed to present after being encouraged by Bonnie, June and Mary Tatiana Haas-Castro of Wake Co. Human Services is knowledgeable about Mexican culture and accepted Mary’s invitation Spring James, who is an African American LC at Rex, accepted Sheri’s invitation to speak Inspirational Dance Finale (after cultural panel): Adonna Simpson has agreed to do an inspirational dance as a finale after the cultural panel and as a send-off for the conference. Diane reports she was pleased to accept a conference registration as her honorarium. Learning Objectives: Thank you to Joan Levy who volunteered to write these objectives: First session: Discuss clinical applications and ethical considerations when utilizing the new WHO Growth Standards Second session: 1. Discuss current research findings related to breast anatomy 2. Assess the therapeutic effect of breast ultrasound to open plugged ducts 3. Describe how reverse pressure is used to soften the engorged breast 4. Discuss the application of nipple vasospasm treatment for persistent sore nipples Third session: Identify possible causes for infant allergies in relation to genetics, breastfeeding, formula feeding and solid foods Fourth session: Can have one general objective or one for each topic: General - Develop a culturally sensitive plan to assist breastfeeding mothers from various cultures OR 1. 2. 3. 4. 5. Describe customs and culture affecting breastfeeding support/education for mothers from Nigeria. Describe customs and culture affecting breastfeeding support/education for African-American mothers. Describe customs and culture affecting breastfeeding support/education for mothers from Mexico. Describe customs and culture affecting breastfeeding support/education for mothers from Ecuador. Describe customs and culture affecting breastfeeding support/education for mothers from China. 3 Proposed Agenda 8:30-8:45 Welcome and Introductions 8:45-10:00 Keynote: The New WHO Growth Standards: Recommendations Miriam Labbok Door Prizes/Exhibits/Networking Break 10:00-10:30 10:30-12:00 Current Practice Issues: New Evidence of Breast Anatomy Margie Mould/Elaina Lee Therapeutic Breast Ultrasound To Open Plugged Ducts Heather Reeves Reverse Pressure Softening for Engorgement Jam Gourley and Michele Hojnacki Nipple Vasospasm Treatment for Persistent Sore Nipples Ann Skinner (15 minutes) (25 minutes) (25 minutes) (25 minutes) 12:00-1:15 Door Prizes/Lunch/Exhibits/Networking 1:15-2:15 Allergies and Breastfeeding Update Karen Dunn 2:15-2:45 Break/Exhibits/Networking 2:45-4:00 Cultural Considerations (10 minutes/5 minutes questions) Nigerian Lois O, WakeMed Mother-Baby Nurse (former midwife in Nigeria) African American Spring James, Rex Healthcare Lactation Consultant Mexican Tatiana Haas-Castro, Wake County Human Services Ecuadorian Betsy Herman, WakeMed Lactation Consultant Chinese May Slowik, Pediatric Resident, Duke University Medical Center 4:00-4:10 Special Dance: A Tribute to Mothers and Babies Adonna Simpson 4:10-4:15 Door prizes/Evaluations/Adjourn Please notice when there is a box at the beginning of a sentence in these minutes that there is an action item - a job that someone said they would do Please bring items for a Spring Basket to raise money for the scholarship fund!
