DIOCESE of JOLIET CHAPTER KNIGHTS OF COLUMBUS Victor Cardosi Award Recognition Dinner 2013 Nomination Form The Joliet Diocesan Chapter of the Knights of Columbus annually holds a Recognition Dinner, for those individuals selected to be recipients of the Victor Cardosi Award. The selected individuals are those who, as Knights, have exemplified Charity, Unity, Fraternity and Patriotism in Family, Community, Church, Youth and Council Activities above and beyond that of the average council member. Chapter Officers are exempt from receiving this award. The 2013 Victor Cardosi Award Recognition Dinner will be held on Saturday, April 20, 2013 at Joliet Council # 382 in Joliet, Illinois. Nomination RULES Nomination will only be accepted from Grand Knight of Council If Grand Knight is being nominated, than Deputy Grand Knight or Financial Secretary Nomination submissions should use a 12 point Times New Roman font. CHAPTER DUES MUST BE PAID PRIOR TO OR NO LATER THAN THE DEADLINE FOR NOMINATIONS OF FEBRUARY 08, 2013 All nominations must contain all eight (8) sections described on the following pages. Each section is to be numbered and labeled appropriately (i.e. Section 1 – Information Page, Section 2 – Service to Church and Parish …) For each section, identify activities or characteristics the nominee holds beyond that of an average council member. All nominees must be a member of the Knights of Columbus. Each council may submit only one entry per year from their Council. Previous submitted nominees, may be re-submitted only by using this form (with all 8 sections). Nominations for older members who were very active members in the past and are less active now should be submitted for consideration. No nominations, for deceased members will be considered. Nominations containing inaccurate or misleading information may be eliminated from consideration by the selection committee.
