An invitation is extended to you to submit a written quotation for the following:
QBSC/14/042 – Excavation, Crushing and cartage of gravel from Tudehope’s Pit to the Burdekin Shire
Council, Jones Street Depot.
Quotations must be in accordance with the attached Specification and Criteria. Please ensure your
written Quotation is lodged prior to the Closing Time, being 12 noon on 10th April 2015.

Insurance
The successful Respondent and any subcontractor(s) will be required to effect
and maintain insurance policies in the following sums:
(a)

any one occurrence and for an unlimited number of claims.
(b)
Workers Compensation or Personal Accident Insurance cover as required
by law (whichever may apply).
(c)
Product liability insurance in the sum of at least $200,000 in respect of any
one occurrence and for an unlimited number of claims.

3.
SPECIAL CONDITIONS
3.1.
Quoted prices
3.1.1
Prices quoted are to be on a Fixed Price. Schedule of Rates Basis..
3.1.2
All prices for goods and/or services offered are to be fixed for the term of the Contract. Quoted
prices must include Goods and Services Tax (GST).
3.1.3
Unless otherwise indicated, prices quoted must include delivery to and unloading at the site
specified in the Specification and all applicable levies, duties, taxes and charges. Any charge not
stated in the Quotation as being additional will not be allowed as a charge for any transaction
under any resultant Contract.
3.2.
Lodgement of Quotations
3.2.1
The Respondent must submit:

(a)
the original of the Form of Quotation;
(b)
completed schedules; and
(c)
written statements and other information (if required).
Quotations must be submitted duly signed and in a sealed package clearly endorsed with the
Quotation Title and Reference Number and must be addressed to:

3.3.
Quotation Closing Time
3.3.1
Quotations close at 12 noon on the 10th April 2015.
3.3.2
Only those Quotations received by the Closing Time will be considered.
3.3.3
Quotations that are mailed will be dated and time stamped when received.
3.3.4
The Local Government will accept no responsibility in the event that a Quotation is not received
by the Closing Time.
3.3.5
Quotations must be in the tender box at the Burdekin Shire Council chambers no later than 12
noon on the date of closing.
3.3.6
Electronic or late quotations will not be accepted.

3.4.
Informal Quotation
3.4.1
Any Quotation may be rejected if it:
(a)
does not comply with the requirements of the Specification or this Request for Quotation;
or
(b)
contains any provisions not required by this Request for Quotation.
3.5.
Acceptance of Quotations
3.5.1
The Local Government is not bound to accept the lowest or any Quotation.
3.5.2
The Local Government may accept part of a Quotation.
3.5.3
This Request for Quotation together with the Local Government’s written acceptance of the
Quotation, shall constitute the Contract between the Local Government and the successful
Respondent.
3.5.4
The Respondent agrees that the Quotation will remain open for acceptance for a minimum period
of 30 days after the Closing Time, notwithstanding that there may have been negotiations in
respect of any Quotation in the meantime.
3.5.5
A Respondent may withdraw its Quotation at any time after the expiration of 30 days from the
Closing Time, but must not withdraw their Quotation prior to the expiration of 30 days from the
Closing Time.
3.6.
Contract Duration
3.6.1
The Contract will be in force for the period of 6 months commencing on the date of the Local
Government’s written acceptance of the Quotation.
3.7.
Canvassing of Elected Members and Council Officers
3.7.1
Canvassing of elected members or Council officers will automatically lead to disqualification.
3.8.
Indemnity
3.8.1
The successful Respondent must indemnify and keep indemnified the Local Government against:
(a)
any liability under the Workers’ Compensation and Rehabilitation Act 2003 as amended
from time to time; and
(b)
any other law in force or which during the term of this Contract may come into force, under
which any person is entitled to claim or sue for compensation or recover any damages
from the Local Government.

4.
SPECIFICATION
4.1.
Introduction
4.1.1
The quotation is for establishment of equipment to Burdekin Shire Council’s Tudehopes Pit site,
excavation, crushing, stockpiling at site and/or transportation of crushed material to Burdekin
Shire Council’s Jones Street Depot, Ayr and disestablishment from site.
4.1.2
The site of works is Burdekin Shire Council’s, Tudehopes pit located on Lot 1 on GS 126.
Tudehope’s pit is 60km by road via the nominated haulage route from the Burdekin Shire Council,
Jones Street Depot.
4.2.
Hours of Work
4.2.1
The hours of work shall be from 6am to 4pm Monday to Friday with the last delivery to the Jones
Street yard being no later than 4pm, excluding public holidays.
4.3.
Insurances
4.3.1
Copies of all requested insurances are to be provided with the quotation.
4.4.
Quarry License and Royalties
4.4.1
Council has a current quarry license for the site and council will pay all royalties directly to the
Queensland Government
4.4.2
Council shall also pay Portable Long Service Leave
4.5.
Workplace Health and Safety
4.5.1
Tenderer is to provide a copy of their Workplace Health and Safety Management System for
review by Council
4.6.
Crushing and Screening of Material
4.6.1
All material shall be crushed and screened to a maximum size of 30mm
4.6.2
Dust suppression shall be maintained at all times during crushing and the haul roads shall be
watered 4 times a day (minimum) whilst hauling material.
4.6.3
The haul road shall be graded, watered and rolled, to restore loss of any formation and eliminate
corrugations, from the Ayr/Dalbeg Road to the pit floor at the end of the works. Hauling and
crushing during wet weather will be at the discretion of the Burdekin Shire Council.

4.7.
Nominated Haulage Route
The nominated haulage route for the contract is as follows: after exiting the pit follow Tudehope
Road to it’s intersection with the Ayr/Dalbeg Road. Continue along the Ayr/Dalbeg Road to it’s
intersection with the Bruce Highway at the United Service Station. Continue along the Bruce
Highway to the roundabout at Don West Toyota and then along the heavy vehicle bypass to
Council’s Jones Street Depot.
4.8.
Material Being Delivered to Jones St. Depot
4.8.1
The principle shall be provided with a printout from an approved Loadrite or similar weighing
device for each load.
4.8.2
A current certificate of calibration for the approved Loadrite or similar weighing device being used
shall be provided to council prior to the commencement of works.
4.8.3
Council will require 4 loads per day to be weighed by a certified weighbridge. Loads will be
randomly selected from the trucks during the day. The tare weight of all trucks will be established
prior to the contract commencement and fuel tanks shall be full when the tare weight is taken.
Council may inspect the tanks at that time. It will be the contractors responsibility to organise and
pay for all weighbridge certificates required by the contract and copies are to be provided to the
principle.
4.8.4
Burdekin Tyre Service, (07) 4783 2500, are prepared to provide weighbridge certificates between
the hours of 8 am to 4 pm Monday to Friday. Burdekin Tyre Service is located at 4/71 – 85 Home
Hill Road, Ayr which is on the nominated haulage route.
4.9.
Material Being Stockpiled on Site
4.9.1
The principle shall be provided with a printout from an approved Loadrite or similar weighing
device for each load.
4.9.2
A current certificate of calibration for the approved Loadrite or similar weighing device being used
shall be provided to council prior to the commencement of works.
4.9.3
The location of the stockpile shall be identified by the principle at the mandatory site inspection.
4.9.4
The unit of measurement shall be per tonne as measured by the approved Loadrite or similar
weighing device.
4.10. Quantity of Material and Plant.
4.10.1 The quantity of material shall be in the order of 13,000 tonnes delivered to the Jones St Depot
and 2000 tonnes stockpiled at the Tudehopes Pit site. These quantities may be varied by up to
500 tonnes in each case depending on the quoted price.
4.10.2 The tenderer shall provide a full list of all Plant being utilized during the works in their cover letter.

4.11. Payment
4.11.1 Payment shall be via monthly invoices submitted to Council and approved by the principle.
4.11.2 Payment will be calculated by tallying submitted Loadrite or similar weighing device dockets and
multiplying by the successful contractors tendered unit rates inclusive of GST for each dump site.
4.11.3 Payment for the establishment and disestablishment to site will be made at the completion of the
contract.
4.12. Material Grading
4.12.1 The successful tenderer shall provide N.A.T.A. approved certificates of the crushed material
gradings similar to the particle size distribution report from S.E.S attached to the “Request for
Quotation”
4.12.2 One certificate shall be provided at the commencement of works and prior to delivery of any
material. A further two certificates shall be provided when directed by the Principle during the
delivery of the material.
