For admission checklist
We are pleased that you are considering applying to the University of WisconsinBaraboo/Sauk County. This checklist will help you in completing your application in
an efficient and timely manner. If you have any questions, please contact the Office

ADMISSIONS
Application
Complete and return the UW System Application for Undergraduate Admission
to the Office of Student Services.
Completing the electronic version of the UW System application is recommended. To apply
electronically, type the address in the location box of your browser: http://apply.wisconsin.edu/ or visit
the UW Baraboo/Sauk County website and click on the “Apply Online” link. Applicants without
convenient internet access may contact the Office of Student Services for a paper application form.

This fee is required of all degree seeking applicants who have not been previously enrolled at a UW
College (freshman/sophomore) campus. Make your check payable to UW-Baraboo/Sauk County.
High School & Any
College Transcripts
Transcripts
If you are a high school student, submit your completed application and a $44 fee to your high school
guidance office. Request your counselor to submit an official high school transcript along with the
completed application and $44 fee.
If you are already out of high school, submit your completed application to the Office of Student
Services and contact your high school to have an official transcript forwarded.
If you attended another college or university, submit an official transcript in addition to your high
school transcripts.
Individuals who have completed or are completing the GED or HSED will need to provide
official scores and high school transcripts.
Submit either ACT
or SAT score
ACT or SAT Scores
Either an ACT or SAT score report is required of all new freshmen applicants 21 years of age and
under (ACT preferred). Official copies sent directly from ACT or SAT are preferred; however, a
photocopy of the score report or a copy of an official high school transcript that includes the ACT or
SAT score information is acceptable. Exceptions to this requirement must be approved by the campus
Assistant Dean for Student Services.

Pre-enrollment checklist
Placement Testing
All new freshmen must take the placement tests in the areas of English and Mathematics. An optional
foreign language placement exam is also available. Transfer or non-degree students must also take
these tests if they have not successfully completed a previous university level English or math course.
The results are used in determining appropriate course placement in these academic areas. There is a
placement test fee assessed to any students admitted as new freshmen. Testing information will be
provided with the admission letter.
Registration
Information regarding registration dates will be provided with the admission letter. Registration will be
held in the summer for new freshmen and new specials for the fall term, and in January for the spring
term. Transfer and re-entry students may register on the continuing on general registration dates. These
dates are published in the campus timetable of courses.
Orientation
Orientation is required of all new freshmen. Freshmen should plan on attending one of the SOAR
programs, which includes opportunities for receiving information, advising, and registration.
Financial Aid Checklist
Financial Aid Application
Complete the Free Application for Federal Student Aid (FAFSA).
Forms are available at the high schools or from UW-Baraboo/Sauk County.
It is recommended that individuals with convenient internet access complete the FAFSA form online.
You may apply on-line at the following address: http://www.fafsa.ed.gov . Make sure you have the
correct browser if applying on-line. The Department of Education provides assistance to WEB users
through a Customer Service number: 1-800-801-0576. Other questions regarding your FAFSA
submission may be directed to 1-800-433-3243. For those without internet access a paper copy of the
FAFSA form may be obtained from either your high school or the campus Office of Student Services.

List school exactly as: UW 2 yr Campuses; Madison, WI 53708
Use the completed tax information from the previous year when completing the application. If possible
DO NOT complete an application based on estimated income information.
File Early
Apply for financial aid early. Applicants who have submitted all necessary forms by April 15 will be
given funding priority. Many aid sources are limited. All application materials should be submitted as
soon after January 1 as possible for the academic year beginning the following September. Processing
generally takes 2-3 months.
Save
Keep a complete file of all financial aid papers! When receiving requests for additional information,
please respond quickly. If you have questions after receiving your Student Aid Report (SAR) or your
actual Financial Aid Award Offer, contact the Office of Student Services at UW-Baraboo/Sauk Co.
