Applications are invited for a full-time post in Psychotherapy in South Clyde. The post is
based at Dykebar Hospital with additional sessional input to Inverclyde. On call
responsibilities will be shared with other consultant colleagues on the South Glasgow
and Clyde rota.
The current work main components are clinical and training, including assessment of
patients, individual and group psychoanalytic psychotherapy, psychodynamic
counselling, consultation to other professions and supervision, teaching and training of
clinical staff. The consultant will join a dynamic team of 4 therapists with dedicated
secretarial support.
The successful candidate would be encouraged to take the opportunities available for
further education and continuing professional development. There are active education
and audit programmes within the service.
Applicants must have full registration with the General Medical Council, a licence to
practice and be eligible for inclusion in the GMC Specialist Register. Those trained in
the UK should have evidence of higher Specialist Training leading to a CCT in
Psychotherapy or eligibility for specialist registration (CESR) or be within six months of
confirmed entry at the date of interview. Non UK applicants must demonstrate
equivalent training.

JOB DESCRIPTION
1.
THE POST
The post holder will be a Consultant in the Dykebar Psychotherapy service and will work
with other members of the team to provide the psychotherapy service for South Clyde.
The post-holder will be expected to maintain and develop links between the
Psychotherapy Clinic and the General Psychiatric Services (both inpatient and
outpatient). In addition his or her remit will include being Psychotherapy Educational
Supervisor for the psychiatric trainees on the South Clyde rotation.
The post holder will be required to have training in psychotherapy and will either have a
CCST in Psychotherapy (or equivalent specialist training and experience) or a CCT in
Psychiatry and a commitment to undertake equivalent training in order to apply for entry
to the specialities register as a psychotherapist.
The Psychotherapy Service in South Clyde forms part of the overall psychiatric provision
for South Clyde (which has a population of 270,000) in Renfrewshire and Inverclyde.
The work carried out in the clinic is underpinned by psychoanalytic thinking, although
this job would not preclude someone from a different psychotherapeutic background
such as Cognitive Behavioural therapy applying for this job. The department aims to
provide a specialist service whilst also forming good links with general mental health
services to foster psychological thinking in all mental health settings. It is anticipated
that the psychotherapy department will strengthen existing links with the psychology
department in future.
Its main components are clinical and training. The small team of 4 non-medical
therapists offers assessment of patients, individual and group psychotherapy, individual
psychodynamic counselling, consultation to other professionals, and the supervision,
teaching and training of clinical staff including junior doctors and senior psychiatry
trainees. Since 2008, the psychotherapy service has been developing expertise in
delivering mentalisation based treatment for patients with borderline personality
disorder. Referrals are received from the general psychiatry service, psychology service
and from general practitioners. It also provides input through consultation and
supervision to the general psychiatry work of doctors, nurses and AHPs in South Clyde.
2.
PSYCHIATRY AND PSYCHOTHERAPY SERVICES
Psychiatric services in Glasgow & Clyde are divided into four geographical areas
(North/East, South, West and South Clyde). There are five psychotherapy services, one
each in North, East, South and West. There is also a Consultant Psychiatrist in
Psychotherapy leading the Glasgow wide “Homelessness Personality Disorder
Service”. The Consultant Psychotherapists from each of the Glasgow services meet as
the Division of Psychotherapy and together, with other Consultant Psychotherapists in
the West of Scotland, form a network of colleagues who meet regularly to discuss
clinical matters.
In terms of psychiatric provision the South Clyde covers a population of about 270,000
including areas of significant deprivation. The Psychotherapy service based in Dykebar
have always had a responsibility to provide a service to Inverclyde, but traditionally the
service has emphasised its role on Renfrewshire, with input to Inverclyde being a
relatively small part of the total workload.
The psychotherapy service, working jointly with psychology provides the Inpatient
Psychological Therapies service (IPTS) to the acute admission & rehabilitation &
recovery wards within Dykebar Hospital. This is an innovative project, which has been
much welcomed by the general psychiatry service. The two nurse therapists from the
psychotherapy department to provide an assessment service for patients within the
acute psychiatric admission wards. They have also been involved in training, supporting
& supervising nursing staff in the delivery of low-intensity psychological intervention and
in developing reflective practice groups and encouraging the development of
psychological
Secondment opportunities for other members of the mental health service have been
developed to enable them to rotate through the psychotherapy department to gain such
experience.
There are well developed community psychiatric services with general adult provision
from provided by Community Mental Health Teams based in three Community Resource
Centres (one in Inverclyde and two in Renfrewshire). There are currently in-patient units
in two bases; Langhill Clinic in Inverclyde and Dykebar Hospital in Renfrewshire.
Inpatient psychiatric services in Renfrewshire are provided at Dykebar Hospital, which
has 42 acute inpatient beds at present. The hospital has access to a total of 8intensive
psychiatric care unit beds based in Inverclyde. A total of 20 beds provide inpatient
intensive rehabilitation and continuing care services for South Clyde.
The psychotherapy service has developed close links with the community services in
Renfrewshire, working with both community mental health teams and the Intensive
Home Treatment Team (IHTT). As well as facilitating reflective practice groups within
these teams, the psychotherapy service has been supporting the development of
mentalising-based treatment skills in staff, to help in the management of patients with
borderline-personality disorder.
The psychotherapy service has developed close links with the Renfrewshire primary
mental health care team (Doing Well). One of the senior members of the psychotherapy
service has been consulting to the team and conducting psychotherapy assessment
within the team for 2 sessions week since 2008.
3.
DUTIES OF THE POST
The 10 programmed activities will be divided to give 9 programmed activities for direct
clinical care and 1 programmed activities for supporting professional activities. In line
with all other consultants in the area 1 programmed activity is nominally identified as
group task time. This is time in which work done for the whole area is timetabled – group
tasks being such things as clinical tutor, managing a long stay ward etc. The group task
is a flexible responsibility that can be changed through job planning – in this post the
initial group task identified is the provision and coordination of psychotherapeutic training
for trainees. In association with the Clinical Director, Dr A Thom the post holder will be
responsible for providing and coordinating the psychotherapy services in South Clyde.
This will involve:

1. The detailed assessment of patients referred to the clinic, and the planning
of their treatment. This will include supporting the weekly meeting of clinical
staff held to discuss patients in assessment, to encourage a thorough
understanding of the patient, formulation of their difficulties and
consideration of appropriate further treatment and management.

Treatment of patients.

Supervision of the clinical work of service staff (medical and non-medical).
4.
Offering expert advice to colleagues on clinical issues.
Education & Training
The post holder will be the Educational Supervisor for junior doctors training in
psychotherapy on the South Clyde Psychiatry Rotation as group task. He or she
will contribute to the training of junior psychiatrists who are required to meet the
Royal College of Psychiatrists mandatory guidelines for training in psychotherapy.
Currently junior doctors on the training scheme come for weekly supervision in
small groups of their individual psychotherapy work with patients. The service
provides a weekly Doctor-Patient Relationship Seminar where trainees discuss
aspects of their general psychiatric work from a psychodynamic perspective.
Additional supervision will also be provided by Dr Cohen and other senior
members of the clinic staff. At any one time there is usually one SHO on a 6
month placement in the department; their duties during this placement would also
include some CMHT work.
A significant focus of future development will be for psychotherapy to help train,
support and supervise other mental health professionals in their psychological
interventions within their owns areas of practice. IN particular, the service will
continue to focus on the development of mentalising based treatment skills.
In time the post holder may also have training input to senior trainees (ST4-6) on
the West of Scotland Psychotherapy Training Scheme.
It is expected that the post holder will be actively involved in the teaching of
undergraduate medical students of the University of Glasgow. In recognition of
this the University may later on individual application grant the status of honorary
senior lecturer. The post holder will also be expected to take part in post graduate
teaching of psychiatrists in training, trainee general practitioners and on
occasions other disciplines.
The post holder will be a member of the Psychotherapy Subcommittee of the
West of Scotland Committee in Psychiatry which oversees the psychotherapy
component of psychiatry training at all levels.
c)
Liaison
Close links between the psychotherapy clinic and psychiatry are well established
and valued. The post holder will be expected to maintain these, for example by
attending and taking part in case conferences, journal clubs, and other teaching
organised through the general psychiatric service.
The post holder will offer consultation to colleagues about difficult clinical
situations. In addition the post holder will be expected to develop these links by
looking at the interface between the psychotherapy clinic and the psychiatric
services. The particular way in which this might occur would be influenced by the
interests and expertise of the post holder together with the needs of other
services.

On-Call
The post holder will participate in the South Clyde on-call rota for psychiatry on a
pro-rata basis. To allow this the post holder must have completed Section 22
training of the Mental Health (Care and treatment) (Scotland) Act and be an
approved medical practitioner (AMP).

Management
The post holder will be expected to provide medical leadership to the
psychotherapy team and assist with the further development of the service. He or
she will undertake the administrative duties associated with the care of patients
and the proper functioning of the clinical team.
The post holder will be expected to participate in the redesign of mental health
services currently taking place in Renfrewshire. The development of improved
community based services provides an opportunity to enhance the input of
psychological therapies in this area. Work continues to improve the therapeutic
environment in acute in-patient services, and continuing psychotherapy
involvement in this programme.
The postholder will be expected to participate in the medical advisory structure as
a member of the Clyde Division of Psychiatry and other committees as may be
relevant from time to time. He or she will also be expected to represent the
psychotherapy team at clinical management.

Continuing Professional Development
If the post holder does not yet have a CCST in Psychotherapy he or she will be
required to undertake the further training necessary to apply for inclusion in the
Royal College of Psychiatrists Specialist Register as a Psychotherapist. The
precise nature of this training will be agreed in conjunction with the
Psychotherapy Training Programme Director and local Clinical Director, be part of
the job plan, and monitored through the Annual Appraisal and CPD mechanisms.
Funding may be available to facilitate this.
It is desirable that the postholder has completed the basic introductory
mentalising-based treatment training and it would be expected that the postholder
would develop those skills in order both to support the delivery of MBT within the
psychotherapy service and to support other mental health staff in developing
these skills.
The post holder will be expected to maintain good professional standing for CPD
with the Royal College of Psychiatrists. There is a fortnightly meeting of
Consultant Psychiatrists in Psychotherapy from the West of Scotland which is a
forum for the discussion of clinical work and which the post holder will be invited
to attend. There is a theory & practice seminar three times a year for continuing
professional development, which is held in conjunction with the West of Scotland
psychotherapy sub-committee meetings which is invited to attend. There is a
study leave budget and medical staff are encouraged to ensure that they keep
their practice up to date. In addition the post holder will be expected to take part
in the annual cycles of peer review, appraisal and job planning.

Clinical Governance
The post holder will undertake clinical audit and research will be encouraged.
Other involvement in clinical governance will be available eg local committees
and contribution to critical incident investigation.

The basic contract will be for a 10 PA consultant contract. A split of 9:1 between direct clinical
care and supporting professional activities is now the advertised standard for all new consultant
job plans within NHS Greater Glasgow & Clyde. The one SPA minimum will reflect activity such
as appraisal, personal audit and professional development occurring outside study leave time.
Once the candidate has been appointed more SPA time may be agreed for activities such as
undergraduate and postgraduate medical training which takes place outside direct clinical care,
as well as research and/or management. These activities must be specifically and clearly

Employees in the NHS Scheme are “Contracted-out” of the State Earnings
Related Pension Scheme and pay a lower rate of National Insurance
