WORLD HEART DAY 2011 ACTIVITIES PLANNED 1. Title: Screening and risk stratification for a sample of residents of Nadiad town, Gujarat Strategy: Providing the household as a unit of society, an opportunity to be screened for risk factors of coronary artery disease. In keeping with the world heart day message for 2011 we are taking the message of prevention into the home. Rationale: Indo US summit on health recommends that every Indian above 18 years of age should be screened for coronary artery disease. We are in the midst of an epidemic of coronary artery disease and Indians are being affected at younger ages as compared to the established market economy countries. This has huge implications for the healthcare system and the economy of our country. There is an urgent need to increase awareness and enable Indians to live healthy lifestyles. This strategy of offering screening at the home will provide an opportunity to spread message of prevention and will also provide us information on the prevalence of risk factors in this community. Methodology: Demography of Nadiad Population of approx. 2 lakh. Adult population 60%. Average family size is 4. Sample size and method: Stratified systematic random sampling by locality will be done to select a total sample of 500 households i.e ~ 1000 persons. Assuming a prevalence of 10% for the least common risk factor, at least 900 persons need to be studied to comment on the risk factor profile (4pq/d2). Procedure 1. The major areas/locality of Nadiad they reside in will be determined. 2. An awareness talk will be organized in each of these localities which will provide information on coronary artery disease and also the plan for house to house survey and screening. 3. In each area households will be randomly selected for the survey by systematic random sampling. Every 1st, 3rd,5th ……house from the middle of the street/ centre of locality will be included. 4. The interviewer will introduce self and purpose of visit, identify persons in the house eligible to be screened (>18yrs) and obtain written consent to be interviewed, measured and sampled or any combination of the three such as only questions and measurements or only questions etc.. He will follow STEPS questionnaire to obtain details about risk factors and will then proceed to take measurements. Blood samples for sugars and cholesterol will be done on early morning, fasting samples and at the lab. If the subject consents for sampling, a card detailing instructions will be given to him and he will be asked to be ready for sampling between 7 to 8 AM the following day by a lab technician. 5. The list of households with persons for sampling will be communicated to the lab technician at the end of the day. A lab technician will visit the houses at 7 to 8 Am and collect blood samples. 6. The interviewer will be responsible to collect the reports and communicate this to the household with written evidence – card and risk stratification. 7. Data will be recorded into the proforma and submitted to research office for entry daily. Logistics 1. Personnel required 10 volunteers and 5 lab technician Rationale: Average time for an interview ~ 30 mins each. No. of persons to be interviewed ~2000. Timing of survey - 4PM to 7PM. Each day for each interviewer it will be possible to conduct an average of 5 interviews, at least 2 households. 10 persons X 5 each = 50 interviews per day ~1000 interviews in 20 days. 2.
