What do we do? How to contact us Palliative care describes care given to people with chronic, often life -threatening illnesses. We work closely with District Nurses, GPs and many other health professionals to provide expert care and emotional support to anyone who needs support. To arrange a visit or discuss any concerns you may have, you can: Care focuses on improving quality of life, at any stage of the illness from diagnosis onwards. Who are we? The Macmillan service consists of health professionals who have undertaken specialised training to care for patients and relatives who have cancer or other life threatening diseases. Who do we visit? Mainly people who have a diagnosis of cancer – but anyone with a life threatening disease. Patients registered with a GP and living in Manchester and their relatives. How can we help you? The team consists of: Specialist Nurses Occupational Therapist Speech & Language Therapist Dietician Emotional support for patients and carers Advice on managing symptoms e.g. pain, sickness, fatigue, breathlessness Information and advice on all aspects of cancer and palliative care Advice on Macmillan patient grants Ask your GP Ask your District Nurse Ask your Hospital Nurse/Doctor. There will be a Macmillan Team based in your local hospital. Future visits This will be agreed between the patient and the Macmillan Health Care Professional. In some situations, the patient may be discharged from the Specialist Nurse’s care after a particular problem has been solved – but there will always be a contact number available for future use.
