Dear Prospective Student:
We appreciate your interest in The University of Toledo and, in particular, obtaining initial
teacher licensure in the State of Ohio.
The faculty in the Judith Herb College of Education Health Science and Human Service have
designed master’s level programs for initial licensure. In general, the alternative master’s
programs assume students already have baccalaureate degrees for programs that included much of
the arts and science content courses required for licensure.
To begin the evaluation process for a master’s program leading to initial teacher licensure in one
of these programs, you must:
 Submit with this form all unofficial transcripts of higher education course work taken at other
institutions, including transcripts of course work completed at UT prior to 1976.
 If you attended UT after 1976, we can print the UT transcripts for you.
 Submit with this form a copy of any teacher certifications you may hold or have held.
After the Office of Student Services receives this completed application and required
documentation, your course work will be evaluated. Once evaluated, you will then be notified in
writing of the course work you will need in order to be licensed, including all content course
work. The letter you receive with the evaluation will also explain other program requirements,
including those for the master’s degree beyond what is required for initial teacher licensure.
Please note that application to Graduate School is a second required step, should you decide to
pursue licensure. Admission to the Accelerated LAMP (Licensure Alternative Master’s Program)
requires an overall undergraduate GPA of 2.7 (preferred 3.0) or higher and a 2.7 in licensure
content as well as a separate application and interview. We hope this information is helpful to
you in getting started toward teacher licensure.
I understand that this evaluation request cannot be completed unless I have met the
conditions outlined above. This application is NOT an application for graduate school and
does NOT insure admission to a graduate program. Admission to graduate school and
beginning an alternative licensure program will require separate applications should you
desire to proceed. A typed signature will be accepted.
