Biological Science Collegiate Division Fellowship Commitment Form
Initial each of the following to acknowledge that you have read, understand and agree to meet each of
the criteria.
1)
I am registered for and will attend The University of Chicago campus in Chicago, Illinois in the
2015 Fall Quarter._____
2)
I am presently a first year _____ second year _____ Biology Major student. Yes / No Major?_______
3)
Fellows/Students are not permitted to enroll in any classes at the University of Chicago during the 10week fellowship. I will not register for any course(s) during the 2015 10-week Fellowship. _____
4)
Fellows/Students will conduct 10 weeks of full time research under the supervision of the Principal
Investigator from Monday, June 22, 2015 through Friday, August 28, 2015. _____
5)
Fellows/Students will attend an Orientation meeting on Monday June 22, 2015. _____
6)
Fellows/Students will attend and participate in the BSCD Principal Investigator Thursday seminar
series scheduled during the Fellowship program. _____
7)
Fellows/Students attend and present a Research Progress Report during the fifth (or sixth) week of the
Fellowship Program. Fellows will give an oral power point presentation of their research progress. _
8)
At end of the 10 week program, Friday, August 28, 2015, Fellows must submit a Title and Abstract
(via email to Dr. Strieleman, Ms. Kila Roberts and Dr. Zaragoza) for the final presentation at the
Biological Sciences Collegiate Division Research Symposium held in the 2015 Fall Quarter. _____

Attend and present a power point presentation of the 10-week summer Fellowship research at the
Biological Sciences Collegiate Division Research Fall Symposium to be held on Saturday, at the end
of the first week of the 2015 Fall Quarter. The PI is invited to attend and will introduce the Fellow and
their research project at the 2015 Fall Quarter symposium. A Fellow’s full participation requires
that the Fellow present their research and attend the entire/all day symposium, 8:00 am to about
4:30 pm. _____

Any publication resulting from the Fellowship research should acknowledge the “The Biological
Sciences Collegiate Division Research Endowments at the University of Chicago”. _____
11)
Fellows/students that do not FULLY participate as outlined above will be disqualified, dropped
from the BSCD Fellowship program and forfeit the entire stipend. _____
I have read, understand and agree to meet all of the above criteria if I am awarded the BSCD
Fellowship. I acknowledge that failure to meet any of the above the criteria, at any point during the
Fellowship Program, constitutes that I did not fully participate in the program and understand that I
will be disqualified and forfeit the entire stipend.
