EMPLOYED STUDENT NURSE APPLICATION CHECKLIST
Complete in full ESN Application and Competencies Assessment
(Please save your ESN application in Word Format and email to esn@phsa.ca)
Please include an updated resume with your application
Please provide a copy of your clinical evaluation from your Med/Surgical placement. If you have
not completed your clinical placement please indicate expected completion date and submit to
PHSA Talent Acquisition to add to your application. Once your clinical placement is complete
please send your evaluation form to PHSA Talent Acquisition.
Please include in your application a copy of your unofficial transcript.
School Reference is to be completed by a Medical/Surgical Clinical Instructor for Oncology
placement or Pediatric Clinical Instructor for Pediatrics or Perinatal placement.
Alternatively, you may send a copy of your clinical evaluation from your placement.
Email all documents in ONE email to esn@phsa.ca by the deadline of November 30, 2015.

When completing the ESN Application form, please rank your Agency of preference to work
in with a 1, 2 or 3)

If you choose BC Cancer Agency as your first choice, please choose which location is your
preference (Vancouver, Abbotsford, Prince George, Surrey, Kelowna, Victoria)

For BC Children’s Hospital, please rank the your top two areas/units you are interested in
working in (1,2)

For BC Women’s Hospital, please rank the your top two areas/units you are interested in
working in (1,2)
