TENDER SHEET FORMAT
SPECIFICATIONS OF CHEMICALS & AREA/BLOCK
1. Terms and conditions may be read carefully and complied with before submitting
the tender / response to the proposal. These terms and conditions will not be
modified except a written addendum / corrigendum issued by the
Director/RBIPMT. No provision hereof shall be deemed waived until and unless
such waiver is issued in writing and signed by Director/RBIPMT. If any term and /
or condition of this document are held invalid, the remaining document shall
continue to be in full force and effect.
2. The contractor shall use a special combination of chemical products from the firm
of repute.

3. Above mentioned chemicals & regime are not final, if the contractor has
latest/better option he must specify the chemicals along with percentage, quantity
& frequency of application. For any new pesticides/Chemicals mentioned other
than above in the tender, the tenderer will have to provide a certificate issued
form competent authority regarding the safety of such pesticides/Chemical on
Human.
4. If modern equipment are to be used, the list of the equipment to be used is to be
submitted in the tender. Safety in this regard will be sole responsibility of the
contractor.
5. For Mosquito control the firm will be provide 200 Nos. Mosquito machines for
sensitive area like OT, ICU, Private ward, in various wards & also for offices of
officers. Chemical for these machines will be replenished per month or as per
requirement by the firm.
6. All technical information should be submitted in the Technical bid in a plane
manner.
7. The chemicals/materials are to be used should adhere to safety norms issued by
Govt. of India/GNCTD from time to time. No banned chemical should be used by
the contractor. The contractor shall provide pest control services in time to
prevent inconvenience and chemicals (insecticides) used should not be
dangerous to the human being.
8. All material required for these services will have to be arranged by the contractor
and it should be of the best quality not detrimental to the health of hospital staff,
patients or the applicator.

10. This contract will be for one year and extendable for another next one year on
mutual agreement basis of both the parties after expiry of validity of contract.
11. Each document to be uploaded through online for the tenders should be less
than 2 MB in my documents, if any single document is more than 2 MB in my
document, it can be scanned with lesser resolution and the same can be
uploaded. However if the file size is less than 1 MB the transaction uploading
time will be very fast. There is no restriction on the total size of that file
uploaded for the Bid document. The total size of the bid document is based
on the memory available in the total client and the internet band with
available at the time of uploading the bid document.
12. the bidder should see that the bid document submitted should be free from virus
and if the documents could not be opened, due to virus, during tender opening,
the bid is liable to rejected.
13. If any item is tax free then an undertaking should be submitted stating that the
said item is tax free.
14. At least two technician will be deployed by the contractor for Pest & Rodent
Control Services for 8 hours daily (Except Weekly or any other Government
holidays) or as per need. They will report to CMO/Sanitation.
15. The contractor shall not employ any person who has not completed eighteen
years of age.
16. Frequency of measures for controlling the Pest should be once in a week of each
block/Area and as and when required.
17. The company should be ISO 9001 certified. Certificate in this regard should be
submitted by the contractor.
18. The hospital shall not be responsible financially or otherwise for any injury to the
staff by the contractor in the course of performing the duty for and on the behalf
of the contractor.
19. All Pest & Rodent control services in the premises of the hospital will be the sole
responsibility of the contractor. The services will have to be provided at least
fortnightly in each specified areas and on SOS basis. The tenderers are advised
to examine all the area mentioned above before filling the tender..
20. The bidder should submit a Performance Certificate duly issued by the
Procurement Officer/MO I/C Stores/Medical Superintendent/HOD of any Govt.
Hospital with bed strength of 200 or more. The Performance certificate should be
issued within one year before the date of the tender.
21. Payment will be made on monthly billing on satisfactory reporting by
CMO/Sanitation or by any officer appointed by the competent authority.
22. The department will deduct income tax under section 194-c of the Income Tax
Act 1961 from the contractor prevailing rate of such some as Income Tax on the
comprised therein. (Education & other cess as applicable).
23. Inspection will be done by the Director/RBIPMT or his authorized representative.
24. Force Majeure:- If at any time during the currency of the contract, either party is
subject to force majeure, which can be termed as civil disturbance, riots, strikes,
tempest, acts of nature/God etc. which may prevent either party to discharge
their obligation, the affected party shall promptly notify the other party about the
happening of such an event. Neither party shall, by reason of such event, be
entitled to terminate the contract in respect of such performance of their
obligations. The obligations under the contract shall be resumed as soon as
practicable after the event has come to an end or ceased to exist. The
performance of any obligations under the contract shall be resumed as soon as
practicable after the event has come to an end or ceased to exist. If the
performance of any obligation under the contract is prevented or delayed by
reason of the event beyond a period mutually agreed, to if any, or seven days,
whichever is more, either party may at its option terminate the contract.
25. Performance Security: The successful bidder shall furnish Performance
Security to the Department within the time as decided by the department before
signing the contract which shall be equal to 10% of the total value of the Contract
and shall be in the form of an Account Payee DD or a Fixed Deposit Receipt or a
Bank Guarantee Bond from a Nationalized Bank in the performa which would be
valid up to a period of sixty days beyond the date of completion of all contractual
obligations and no interest shall be payable on performance security on its
release.

Sir,
1. The undersigned certify that I have gone through the terms and conditions
mentioned above and undertake to comply with them. I have no objection of the
contents of the tender document and I/We undertake not to submit any
complaint/representation against the tender documents after submission date
and the time of tender. The rates quoted by me/us are valid to and biding on
me/us for acceptance for the period from the date of finalization of the current
tender.
2. I/We undersigned hereby bind myself/ourselves to Director/RBIPMT to supply the
various item for RBIPMT during the period under contract commencing from the
date of finalization to date of finalization of next tender.
3. Bid Security deposited by me Rs. 12,500/- (Rupees Twelve Thousand Five
Hundred only) in the form of FDR/Bank Guarantee in the name of
Commissioner, North Delhi Municipal Corporation as attached herewith and shall
remain in the custody of the Director/RBIPMT till the validity of tender. I/We have
also understood that the bid security deposit may be adjusted in the Post tender
performance Security deposit.
4. I/We shall forfeit to the Director/RBIPMT the bid security in case of any any
unsatisfactory work and conduct or my part or my workers/agents.
5. In case of my liability to perform my contract on the given day or given duration if
the Director or his authorized person gets the job done against my contract, I lwill
be liable to pay expenditure incurred during so by the hospital.
6. The condition herein contained shall form part of the agreement.
7. I/We declare that no legal/financial irregularities are pending against the
proprietor/partner/worker of the tender.
8. I/We undertake the rates quoted by me when approved by the Director/RBIPMT
will be firm during the validity of the tender.
9. Affidavit that no CBI/criminal proceedings/black-listing is pending or going
against me or my worker.
10. I/We undertake that I /We will not submit any irrelevant documents with the
tender and in doing so I /We will not have any objection if my tender is rejected
on that behalf.
