QOF Subset 2014-15 dementia extraction
Opt Out Form for General Practices
The Dementia Challenge, launched by the Prime Minister two years ago, sets out the Government’s
commitment to increase diagnosis rates, raise awareness and understanding and double funding for
research in dementia by 2015. In support of this the Secretary of State for Health has asked the
HSCIC to start collecting dementia diagnosis data ahead of QOF 2014-15, which is scheduled to
begin extractions later this year, and which will contain the dementia diagnosis data.
Please complete the following form if your Practice wishes to opt out of the QOF Subset 2014-15
dementia extraction.
