Q. What does “Voluntary” benefits program mean?

A. Voluntary means that you participate in the program only if you choose to. The cost of the insurance is 100% paid for by the associate.

Q. I want to enroll in the Legal Plan, Auto and Home Insurance, Pet Insurance, and the Fertility Treatment

Discount Program. Where do I find these benefits?

A. You may apply for coverage by calling the Voluntary Benefits Call Center at 1-877-337-6807 .

Q. Who is eligible to participate in the program?

A. All U.S. full-time Office Depot associates are eligible to participate in all the voluntary benefits offered. Part-time associates may participate in Auto and Home Insurance, Pet Insurance and the Fertility Treatment Discount Program.

Q. When are legacy OfficeMax associates eligible?

A. Legacy OfficeMax associates are eligible to enroll beginning with the 2015 annual enrollment period.

Q. Is my spouse or dependents eligible for any of these programs?

A. Yes, all voluntary benefits have some eligibility for spouse or legal dependents. You may call the Voluntary Benefits Call center at 1-877-

337-6807 for more details.

Q. Is there a waiting period for new hires to enroll?

A. There is no waiting period for new hires to enroll for Auto & Home Insurance, Pet Insurance, or Fertility Treatment Discount Program and you may enroll anytime throughout the year. Enrollment in the Legal Plan must occur within the first 60 days of employment or during the annual enrollment period.

Q. I want to enroll in either Accident Insurance, Critical Illness Insurance, Hospital Indemnity Insurance or

Disability Buy-up Insurance. Where do I find these benefits?

A. Full-time associates have access to enroll in these voluntary products on the NetBenefits website ( http://www.netbenefits.com/officedepot ) alongside their core health & insurance benefits.

Q. I am a legacy OfficeMax associate and have Auto and Home Insurance payroll deducted through MetLife or Travelers. Do I need to do anything?

A. No action is required on your part - these deductions will continue going forward.

Q. I am a legacy OfficeMax associate and have Auto and Home Insurance payroll deducted through a carrier that is not MetLife or Travelers. Do I need to do anything?

A. Yes, if you want to continue your coverage, you should pay the direct bill that will be received at your home in January of 2015 as payroll deduction will no longer be available.

Q. How do I pay for these benefits?

A. Premiums are paid through after-tax payroll deductions for full-time associates. Several direct billing options are available to part- time associates. The Fertility Treatment Discount Program is not available through payroll deduction.

Q. Can I keep these benefits if I leave Office Depot?

A. Yes, all of the voluntary benefits are portable or convertible. If you leave Office Depot for any reason you are able to keep your benefits.

Pricing or plan designs may be subject to change for some of the benefits.

Q. Are these benefits sponsored or offered by Office Depot?

A. No, Office Depot has contracted with Arthur J. Gallagher & Co. (AJG) to offer these benefit plans through leading insurance and benefit providers.
