The Meridian Scholarship 2015-2016 Scholarship Application The Meridian scholarship program honors those students who have participated in the PANDA Leaders Club. These students have adhered to their promise to positively influence their peers and continually perpetuate the ideals, goals, and message of the PANDA program. The goal of PANDA Leaders Club is to empower students to take a leadership role within their school and community, inspiring others to form healthy relationships and lead healthy, drug-free lives. A minimum of one $1,000.00 scholarship will be awarded each year. Scholarship may be renewed one time per year for 3 years (4 years total). Applicant must demonstrate good academic standing. Scholarship Requirements 1. Complete and sign the application form (typed). 2. Submit the essay questions typed and as stipulated. 3. Applicant may not be related to an employee of Meridian Healthcare and/or Community Foundation of the Mahoning Valley. 4. Applicant must be a resident of Mahoning, Columbiana, or Trumbull County. 5. Applicant must demonstrate leadership qualities and a commitment to community service. 6. Applicant must have attended PANDA Leaders Club Sr. Camp at one point in their academic career. 7. Applicant must be a high school senior during the 2015-2016 school year, pursuing a two (2) or four (4) year undergraduate or trade school degree. Applicant must plan to attend school in the fall of 2016. 8. Applicant must include two (2) letters of recommendation. At least one (1) must be from a school representative, i.e. teacher, counselor, coach, etc. Recommendation cannot be from a relative. Letters should include writer’s contact information. 9. Application Deadline is January 15, 2016. 10.Recipient of this one-time only scholarship must be able to attend the Meridian Scholarship Recognition Event in early May. (Information on the event will be provided at a later date.) 11.Scholarship Applications will be reviewed by a board/committee comprised of Meridian staff and community leaders. 12. Recipients will be notified by a representative of The Meridian Scholarship in the Spring of 2016. The review board reserves the right to summon applicants for interviews.
