Letter of Consent to Upload Files to an Online Journal Dear Sir/Madam, The History and Theory Department of the Bezalel Academy of Arts and Design Jerusalem R.A. (“Bezalel”) wishes to allow its students, researchers, teachers and the general public convenient access to knowledge in its fields – plastic art, photography, video and animation, visual communication, metalwork, ceramics, glass and fashion, architecture and design, through the new online journal of the department by the name of “Bezalel, Journal of Visual and Material Culture” (the “Journal”). The Journal will be permanently posted on Bezalel’s website, will be published twice a year and will include a collection of articles, visual images and video clips (the “Documents”). The Documents included in the Journal will be available to the general public and will be used by Bezalel for teaching and research purposes. In addition, the Journal will allow readers to download the written text in PDF format. We undertake that the Documents that you shall forward to us and that we shall choose to publish in the Journal shall state their source in accordance with the information that you shall deliver to us. The uploading of documents to the website constitutes consent to all of the provisions above and below. In addition, for the purpose of publishing the article or visual image or video clip, you are asked to sign the letter of consent attached hereto and to return the same to us as soon as possible to fax no.: 02-5893223 or to e-mail: m.visual@post.bezalel.ac.il. I hereby represent that I have not transferred the copyright in the document, have not granted another any part in the copyright, have not given another a license and/or authorization in connection with the said document or in connection with the translation thereof into any language, or with the preparation of any creation deriving therefrom (including in a book, file, reader, newspaper, journal etc.). 3. I hereby transfer to you, without qualification, all of the rights relating to the document, including the copyright, within the meaning thereof in the Copyright Law, 5768-2007 and including the right to publish, modify, edit, publicly perform, make a derivative creation and make the document available to the public, in any manner and at any location and through any means of communication, including on the internet, and including any duplication, within the meaning thereof in Section 12 of the said law, all for an unlimited number of copies and editions, of any size and scope that you shall choose, without any restriction on place, format (including electronic and digital format), language and/or time, and throughout the period of existence of the said rights. 4. I agree to you making any change that is required in the Documents for the publication thereof in the Journal, including the editing, cutting and shortening thereof and changing the size, form, scope and resolution etc. for the purpose of publication thereof. 5.
