Amend the Proposal: P335-15 Employee Drug Testing Laboratory Services
This addendum becomes part of the Proposal Documents and modifies, as noted below, the
original Bidding Documents.
Note 1: The UNMMG does not do any drug testing at this time. They are planning to begin
testing new hires for ambulatory clinics. No decision has been made regarding current
employees. They used Concentra for a drug test in one department.
Note 2: The below listed responses to questions apply to UNMH and UNMSRMC.

Question 3: What is your reason(s) for going out to bid at this time?
Response to Question 3: Tricore has gotten out of the business of forensic employee drug
testing.
Question 4: What are the drugs to be detected on your testing panel and what are the requested
cut-off levels for the testing panel?

The cut-off levels for the testing panel for the purpose of this RFP are UDS 10 panel, we would
like to do the 12 panel if possible. Drugs screened are Amphetamine- Cut off 500ng/ml,
Barbituates - Cut off 200ng/ml, Benzodiazapine 200 ng/ml, Cocaine 150 ng/ml, Ethanol
20mg/dl, Methadone 300ng/ml, Opiates 2000ng/ml, PCP 25ng/ml, Propooxyphene 300ng/ml,
THC 50 ng/ml
Question 5: What was your positivity rate in 2014?
Response to Question 5: UNMH is unable to provide this information at this time.
Question 6: What is your projected testing volume for the requested services? What was the
2014 testing volume?
Response to Question 6: The estimated testing volume is 200 employee tests per year for each
UNMH and UNMMG and 1500 was the estimated 2014 testing volume.
Question 7: Is the vendor required to provide specimen collection services for this bid or are
those services covered under a separate contract?
Response to Question 7: UNMH wants to know what your company offers for specimen
collection services. We want to know what each bidder offers in terms of (1) collection services,
(2) testing services, and (3) MRO services.

Acknowledge receipt of this Addendum in the space provided in Exhibit B. Failure to do so may
subject Offeror to disqualification.
All other provisions of the Proposal Documents shall remain unchanged. This addendum is
hereby made a part of the Proposal Documents to the same extent as those provisions contained
in the original documents and all itemized listing thereof.
