1. For electronic submissions (LHH and MEETH): go to HealthPort, type “internal transfer request form” in the search
box. The link for the Word document will come up. Save the form to your computer. Fill it out and then email it as an
attachment to lhhnursingadmin@nshs.edu.
2.
If you work at MEETH and would like to submit your application manually: submit the completed paper application to
the nursing office. They will scan and email it to lhhnursingadmin@nshs.edu for you.
3.
If you work at LHH and would like to submit your application manually: submit the completed paper application to
“Kirstie Toussaint”. Her mailbox is located in the Nursing Administration Office, 2 nd floor, 100 East 77th Street.
4.
All employees who submit a transfer application will receive an emailed receipt to their NSLIJ email account.
