1. For electronic Submission (LHH and MEETH): go to HealthPort, type “internal transfer request form” in the search box.
The link for the Word document will come up. Save the form to your computer. Fill it out and then email it as an
attachment to nursingadmin@nshs.edu.
2.
If you work at MEETH and would like to submit your application manually: submit the completed paper application to
“Janet Parrales” who will scan and email to nursingadmin@nshs.edu for you. Ms Parrales’ office is located on the 6th
floor, MEETH.
3.
If you work at LHH and would like to submit your application manually: submit the completed paper application to
“Kirstie Toussaint”. Her mailbox is located in the Nursing Administration Office, 2 nd floor, 100 East 77th Street.
