Pharmacist Student Intern
For School Coordinators
1. All changes to rotation assignments must be forwarded to Dr. Monica R. Green no later than two (2) Mondays
prior to the start of the rotation.
This will allow Harris Health System enough time for the following:
a. Provide an accurate orientation list to our Learning Resource Center by the required due date.
b. Assign badge numbers to new students before they begin their rotations with Harris Health System.
c. Provide access to our computer systems on Day 1 of rotations.
2. All Pharmacist Intern File Checklists (aka Immunization Forms) must be completed and submitted to Dr.
Rondell Bailey or Ms. Krystina Smith no later than two (2) Fridays prior to the start of the rotation.




TB skin tests are acceptable for 1 year from the date read. The skin test may not expire before the last day of the
current rotation period.
Chest x-rays are acceptable for 2 years from the date taken. The chest x-ray may not expire before the last day
of the current rotation period.
Background checks must have been completed during the time the student was accepted/enrolled into the
pharmacy program. The indication of a passed background check should be “No Record Found.” Indicate the
date completed as well.
Drug screening must have been completed during the time the student was accepted/enrolled into the pharmacy
program. The indication of a passed drug screen should be “# of panels, Passed.” Indicate the date taken as well.
For Students
Below is the Rotation Process for students at Harris Health System Facilities:
Rotation Day 1 for New Students: Students who have never been to Harris Health System Orientation and Pharmacy
Department Orientation or who have not been to both within the past one (1) year:

New students must complete Harris Health System Orientation online prior to the start of the rotation.

New students must complete Pharmacy Department orientation on-site (Time TBA due to LRC schedule)
at the Staff Development Office on the first floor of Ben Taub Hospital (located behind the outpatient
pharmacy).
After this orientation, students will proceed to the Security Office to be issued Harris Health System Student Badges.
Students will then return to the Staff Development office to sign out and be released to site.
New students must complete both the Harris Health System and Pharmacy Orientation before being released to
the rotation site.
Rotation Day 1 for Returning Students: Students who have completed the Harris Health System and Pharmacy
Department orientation within the past one (1) year:
Returning students report to the Staff Development Office on the first floor of Ben Taub Hospital (located behind the
outpatient pharmacy) (Time TBA due to LRC scheduling) to sign-in and pick up their badges. If assistance is needed in
locating the office, please contact Krystina Smith at 713.873.2045. Returning students will be released to site following
sign in and receipt of Harris Health System badge.
Last Day of Rotation: Last Thursday of Rotation
Students (IPPE and APPE) must go to the Pharmacy Staff Development Office(located at Ben Taub) to sign out and
return their badge and Preceptor Evaluation form on the last Thursday of Rotation (Time TBA due to LRC scheduling).
All students must sign out and return their badge even if they have another rotation scheduled at Harris Health
System. If badges are not returned, the student will not be able to return for future rotations. Note: Times for
orientation, badge pick up/drop off will be sent prior to the start of the rotation.
