Memo To: All Sales: All Front Desk From: Jaime Herrada cc: Pete Hardy, Edie Davis, Bethany McGill, and Tiffany Peche Date: December 6, 2013 Re: Participant Release, Waiver of Liability and Indemnity Agreement (aka guest waiver) Team – Effective immediately ALL guests must sign the liability waiver each and every time, when accessing our facilities. Regardless if they have signed one once before. Waivers are only valid for the actual date signed on the form. By not complying we are put this company in serious legal risk if a non-member injures themselves or others while in our premises. Example: A Walk-in comes in to tour our facility. The WI signs the liability waiver and is given a tour/pricing from an advisor, but wants to come in the next day to tryout a class. He or she would still sign another liability waiver the next day. No Exceptions, No one overrides it. You can direct any questions you may have to me. Thank you for adhering to this policy and for all you do for STW. Jaime Herrada Area Sales Director
