CONSULTANT GENERAL PAEDIATRICIAN
(Neonatal and Ambulatory Care)
Within Combined Child Health Service
Salary Scale - £76,001 - £102,465 per annum (pro rata)
NHS Forth Valley seeks to appoint a Consultant General Paediatrician to an exciting opportunity
within the combined child health service based in the brand new Forth Valley Royal Hospital. This
is a state of the art modern acute hospital with a broad range of clinical services and supporting
departments. You will have a leadership role in the neonatal service and the provision of acute
paediatrics on an ambulatory care basis. You will also be encouraged to have a special interest to
complement the existing consultants, and have opportunities to lead on one or more specialist
areas of the combined child health service including community child health.
The combined department of Child Health currently has a team of 11 Consultant Paediatricians
(General Paediatrics), 2 Associate Specialists, 4 Specialty Doctors Paediatricians, 4-7 Specialty
Trainees, Foundation Year II and GP specialist trainees, an advanced neonatal nurse practitioner
(ANNP) and a medical training initiative (MTI) post. The department provides comprehensive
paediatric and neonatal care to a population of 300,000 people with 60,000 children under the age
of 16 years and 3,200 deliveries per annum. There is a 25 bedded combined medical and surgical
Paediatric ward; a 20 bed Neonatal Unit supporting a consultant led obstetric unit and an 8 bed
Clinical assessment and Ambulatory Care Unit.
Forth Valley paediatric department participates in several regional and national Managed Clinical
networks facilitating the maintenance of specialist interests for all senior medical staff. Forth Valley
paediatric department has teaching commitments to undergraduates and postgraduates from
Edinburgh, Tayside and Glasgow. Involvement in research is encouraged and active participation
in the Scottish Patient Safety Programme (Paediatric, Neonatal and Maternity) is well established.
The primary purpose of this appointment is to participate in the leadership and delivery of the
neonatal and ambulatory care services. The job plan will be ten programmed activities with
additional programmed activities by negotiation dependent upon speciality interest. You will share
in an equal part of the on call rota. You are expected to be suitably qualified and experienced in
general and neonatal paediatrics.

INTRODUCTION TO NHS FORTH VALLEY
NHS Forth Valley is a single, integrated healthcare system comprising acute care facilities and
community based services, which are delivered through Forth Valley Community Health
Partnerships.

NHS Forth Valley is the largest employer in the area, with approximately 8000 staff from a wide

purpose built 860 bedded Forth Valley Royal Hospital, which opened in 2011 and provides

Forth Valley also lies equidistant between Scotland’s two largest cities; Edinburgh and Glasgow.
With their respective airports, both are little over half an hour from most parts of Forth Valley, on
the Falkirk, Larbert and Stirling area lines.

Education in the area is first class, with excellent provision of pre-school, primary and secondary

FURTHER DEPARTMENTAL INFORMATION AND JOB DESCRIPTION
The successful candidate will work within the Combined Child Health service based in either the
Neonatal Unit or the Clinical Assessment and Ambulatory Care Unit and will have opportunities to
provide comprehensive health care for children across acute general paediatrics, care of the
newborn and community based services. Individual consultants may provide general paediatric out
patient services and specialist paediatric services in areas of subspecialty expertise. Consultant
paediatricians will also have the opportunity to take responsibilities for the coordination of services
in the Acute or Community arm of our Combined Child Health Service. There will be extensive
opportunities for teaching and training of junior Medical staff, Nursing staff and Allied Health
Professionals.
The General Paediatrician (neonatal and ambulatory care) post will support the nursing and junior
medical staff to run the neonatal and ambulatory care units. In the past this has included onsite
overnight working and at weekends, but with an innovative new rota designed by medical staff the
requirement for unsupported on site working has been eliminated, providing opportunities for an
excellent work life balance. Duties will cycle through regular weekday working in the paediatric
ward, neonatal intensive care, ambulatory care, and clinic based work in general or specialty
paediatrics. All Consultants are expected to undertake at least one programmed activity per week
of appropriate Supporting Professional Activity (CPD, clinical audit, appraisal, research or service
management duties) as agreed in the job plan.

Actual use of leave depends on contractual entitlement and usual application procedures.
In accordance with department policy, all new consultant posts in our unit include the possibility of
on-site working in support of the middle-grade rota. Such working is not currently on the consultant
timetable and measures are being taken to minimise this possibility. If required, appropriate
notification and job plan accommodation would be made. The successful applicant would be
required to participate in this aspect of out-of-hours cover should the situation arise. The service
required in that circumstance would be as a paediatric registrar; there would be a separate
paediatric consultant on call and available for assistance. We recognise that consultant time is
most appropriately spent in consultant-specific activities and will make every effort to keep the
middle grade rota robust and resilient without requiring consultant working within it.

The General Paediatric Consultants and Associate Specialists are supported by: acute servicesbased Speciality Doctor Paediatricians, community-based Specialty Doctor Paediatricians, training
grade doctors in hospital and community services including general and neonatal paediatrics and a
Tier One resident medical rota comprising 3 GPVTS – training for General Practice and5
Foundation Year 2 doctors. There is also 1 Medical Training initiative posts.
Administrative Support
The successful applicant will have appropriate secretarial support and office accommodation
including a computer with access to the NHS network.
Acute Paediatrics
Hospital-based medical paediatric services are provided on a single inpatient site at Forth Valley
Royal Hospital. This includes: general paediatrics, ambulatory and day case investigations unit,
neonatal intensive and special care baby unit, paediatric surgery from age 3 years, and
anaesthetics (general surgery, urology, ENT, ophthalmology, orthopaedics), paediatric accident
and emergency service, and a number of radiography services for children (including plain X-ray,
contrast studies, Isotope scans, ultrasound, CT and non-sedated MRI).
Inpatient General Paediatrics
Approximately 5000 children are admitted every year. As well as standard inpatient beds there are
also facilities for close observation and stabilisation for transfer to PICU in Glasgow or Edinburgh.
The average length of stay for our admissions is less than 1.1 days demonstrating our commitment
for children to be cared for at home whenever possible. These new posts will further support this
approach. The Consultant Paediatricians share a regular rota to provide continuous support and
supervision to the inpatient children’s ward, neonatal unit, and ambulatory unit during normal
working hours Monday to Friday. This is achieved by having a consultant of the week. This same
group of Consultant Paediatricians and one Associate Specialist also share the on call rota for outof-hours cover for the paediatric service at nights and weekends

Ambulatory Paediatric Care
Around 2000 children per year are seen in the ambulatory care unit or are admitted as day-cases.
The service is in a separate area adjacent to the children’s ward.
Care of the Newborn
Over 3000 babies are born in Forth Valley Royal Hospital each year. The existing Consultant
Paediatricians and one Associate Specialist also have neonatal and ambulatory care skills and will
share the weekly rota to provide continuous support to the antenatal services, labour ward,
neonatal intensive care unit and the postnatal wards. These Consultant Paediatricians also share
an out-of-hours nights and weekends on-call rota and will provide the regular daytime weekend
cover to the neonatal and ambulatory care units.
Neonatal care in Forth Valley is provided at BAPM Level 2 and within the West of Scotland
Neonatal Managed Clinical Network with shared clinical guidelines, audit and education activities
well established. Planning for at risk deliveries or postnatal need for ongoing intensive care is
coordinated through the Perinatal Advisory Service which is currently regional but may extend to a
national service in the future.
Outpatient Clinics
Around 1700 new medical paediatric patients and between 5000 and 6000 return patients are
assessed every year at the outpatient clinics in Forth Valley Royal Hospital, Stirling Community
Hospital and Clackmannanshire Community Hospital. Consultant Paediatricians provide general
outpatient clinics and specialist clinics in neurology, respiratory, endocrine, diabetes, Cystic
Fibrosis, gastro-enterology, and neuro-development. Tertiary clinics are also provided locally by
visiting consultants from Glasgow (cardiology, nephrology, rheumatology, clinical genetics, cleft lip
and palate). Cystic Fibrosis care is shared with the Lothian CF team as part of a national MCN.
Community Children’s Nursing
Seven community children‘s nurses are attached to the department providing out of hospital care
to children with chronic disease and life limiting conditions. One nurse is a complex care sister
(1wte), 4 nurses are community outreach sisters, and two nurses are diabetes liaison nurses (1.5
wte).
NHS Forth Valley has embraced the principles of GIRFEC (Getting it Right for Every Child) and the
Early Years Collaborative that supports this approach to coordinating and integrating the services
that provide for infants and children’s needs.
Community Child Health
The department covers a population of 60,000 children under the age of 16 years. The population
of Forth Valley lives mainly in the city of Stirling and several large towns (Falkirk, Alloa,
Grangemouth, Bo'ness) as well as the rural areas of west Stirlingshire. The living standards in
Forth Valley are high with small areas of severe deprivation. The region has diverse ethnic
backgrounds, but with no established ethnic communities.
The department of community child health works closely with the education and social services in
the three local authorities. The department of community child health operates within the combined
child health service and offers the following services under supervision and co-ordination of
consultant paediatricians.
Disability & Additional Needs
Services for children with disabilities are provided from within multi-disciplinary teams that operated
at different localities. No formal Child Development Centres are operational yet, but plans have
been discussed.
Services are also provided for neonatal follow up of babies with increased risk for disabilities and
also for adolescents in transition to adulthood. Early assessment teams are in place for multidisciplinary approach to children with suspected developmental problems. The department also

works closely with National Children's Homes locally and the two children’s hospices in provision of
services to children in respite care.
Specialist Clinics
Services include children with constipation, encopresis, enuresis, behavioural problems, autism,
attention deficit hyperactivity disorder, feeding disorders, hearing impairment, visual impairment,
prevention of SIDS and sexual health.
Child Protection
All Consultant Paediatricians provide early assessment and examination of children, supported by
nurse advisors. One Consultant Paediatrician will take the role of clinical co-ordinator for child
protection and takes responsibilities for compiling agreed protocols and guidelines. This consultant
will represent the department in the managed clinical network for west of Scotland. The
paediatricians have a close working relationship with forensic medical experts (FME) in Forth
Valley. The FME’s have experience in child sex abuse examination working with the paediatric
consultants who have undertaken additional training in CSA assessment.
Child Health Screening
The department carries out electronic recording of child health screening data, and continues to
undergo developments in line with national recommendations.
Health Promotion and Disease Prevention
The department is involved in the primary immunisation programme by setting local guidelines and
advice to general practitioners and health visitors. The department works closely with the Health
Promotion team at NHS Forth Valley and the Local Authority's Community Schools.
Adoption and Fostering
One Consultant Paediatrician is designated medical advisor for each of the Local Authorities’
Resource panels and provides assessment and reports on accommodated children. The
department also provides health and medical input to “looked after children” in local authorities'
accommodation and also for the adoption and fostering services. A nurse-led service for Looked
After Children has been developed and co-ordinates services with partners in the local authorities.
It has set up an effective system for information exchange, monitoring, evaluation and follow-up,
based on the established practice of health assessments for individual children.
School and Public Health
With the changes for school health as envisaged by the Hall 4 report, changes are already
underway in Forth Valley and public health nurses in schools are been given the training for their
future roles.
Relationship with other departments
CAMH services are based within the Mental Health Unit there are 3 Consultant Child Psychiatrists
providing an area service, who also occasionally utilise in-patient paediatric beds in conjunction
with the paediatric consultants. Separate paediatric clinical psychology staff are also based the
hospital, working in close association with child psychiatry. There is a close link with regional
educational psychology services.
There is a close relationship with the Royal Hospital for Sick Children, Glasgow, our principal
tertiary referral centre for childhood haematology/oncology, neonatal surgery, cardiology and other
supra-regional services. Close working and networking with this tertiary centre and with RHSC
Edinburgh is encouraged.

Teaching and Training and research
NHS Forth Valley places a high priority on training and development across all staff groups and
strongly supports the continuing medical education programme for Consultants. The postgraduate
centre is located within the main hospital building and is well-equipped.
The library contains:

 a comprehensive stock of teaching videos including programmes for the FACS and MACP
examinations
 Medline on CD-ROM for literature searching
 Internet access
 Teleconferencing links for weekly postgraduate teaching from RHSC Yorkhill and Ninewells
Hospital Dundee
 portable colour laptop PC for use with overhead projection
Two specialty registrars are attached to the department from the East of Scotland deanery for core
paediatric training. The department runs a weekly teaching programme. Specialty Registrars (and
Foundation Year 2) posts within the West of Scotland deanery provide training for General Practice
Registrars and trainees in paediatrics to middle grade level. Individual teaching in preparation for
DCH and MRCPCH examination is offered to all trainees. Forth Valley has hosted the MRCPCH
examination on an occasional basis. Three GPs with Special Interest in Paediatrics have been
attached to the Forth Valley Department as the NES GP Paediatric Fellowship. Forth Valley has
supported the training of midwives in examination of the newborn and has established the NES
Scottish Maternity training course in Resuscitation of the Newborn. In June 2014 the first APLS
course in Forth Valley Royal Hospital took place. The Scottish Clinical Simulation Centre is
collaborating with our department in developing additional training opportunities in acute paediatric
and neonatal clinical management.
Training of senior neonatal nurses as advanced neonatal nurse practitioners is underway. A
similar programme for training existing paediatric nurses as advanced paediatric nurse
practitioners is also in place.
Undergraduate Students from the University of Edinburgh are attached regularly for their child
health block and students from University of Dundee attend for special study modules.
Staff are encouraged to participate in research relevant to their work both as lead investigator or
contributing to multi-centre research and to support colleagues who are so engaged.

CONSULTANT GENERAL PAEDIATRICIAN (Neonatal and Ambulatory Care)
RESPONSIBILITIES

As agreed with the General Manager and Associate Medical Director (Women, Children
and Sexual Health Services) to provide with Consultant colleagues an area wide service in
General Paediatrics with responsibility for the prevention, diagnosis and management of
illness, and for the proper functioning of the service;

Out–of-hours responsibilities for Forth Valley Royal Hospital shared with the other
Consultants as part of a Consultant team;

Participate in leading the provision of specialist Neonatal and Ambulatory Care Services at
Forth Valley Royal Hospital including the supervision and training of medical staff, nurses,
midwives and allied health professionals including flexible out of hours working

To provide cover for Consultant colleagues during annual and study leave or at such other
times as agreed with the General Manager and Associate Medical Director (Women,
Children and Sexual Health Services (prospective cover arrangements apply equally to all
consultants)

To offer professional supervision, training and appraisal of junior medical, nursing and
technical staff as appropriate to enhance role extension and job skills within the Paediatric
service in order to improve the service provided to patients;

To motivate staff within the service, through leading by example, and fostering good
working relationships at all levels in line with the principles of the local Partnership
Agreements;

To contribute to developments and initiatives within the organisation as appropriate and as
requested;

To undertake teaching, accreditation and examination duties, and contribute to
postgraduate and continuing medical education activity across the area service;

To agree and implement an annual medical audit and clinical governance programme with
the Associate Medical Director (Women, Children and Sexual Health Services) in line with
the Board’s Clinical Effectiveness Strategy;

To participate fully in Consultant appraisal and personal development planning activities;

To ensure the efficient and effective use of the organisation’s resources;

To work with colleagues to ensure the effective and efficient development and
implementation of support systems.

To support the Paediatric Department Clinical Governance agenda

Operate within the scope of agreed policies in Women, Children and Sexual Health
Services Unit.
The requirements of this post will change through time in line with clinical advances and
developments in services provided by the NHS in Forth Valley.

CONTRACTUAL DETAILS
Contracted activities for this appointment will be defined and agreed with the post holder and the
General Manager, Women, Children and Sexual Health Services Unit (WCSHSU).
The post holder will be professionally responsible to the Associate Medical Director, SCCSU and
managerially responsible to the General Manager, WCSHSU.
The Consultants in the department will agree arrangements for cover of General Paediatric duties.
This Consultant post is available on a full-time basis but the Board welcomes applications from
individuals who are unable to undertake a full time commitment and who may wish to work on
either a part-time or job share basis subject to satisfactory proposals to meet the needs of the
service.
The terms and conditions of service applicable to this appointment are the Consultant grade terms
and conditions of service as amended from time to time.
NHS Forth Valley will require each Consultant to agree with the General Manager and Associate
Medical Director or Lead Pediatrician a job plan of fixed and other commitments, which will be
reviewed on an annual basis and revised as necessary to meet the requirements of the local and
national service issues.
NHS Forth Valley does not negotiate salary placements. On commencement of employment,
salary will be determined in accordance with Section 5 of the terms and conditions of service.
Consultants will be required to participate in an annual appraisal.
The appointment will be superannuable under the NHS Superannuation Scheme, which is
contracted out of the State Earnings Related Pension Scheme (SERPS). New employees will

The successful candidate will be expected to be registered for CPD with the Royal College of
Paediatrics and Child Health and to fulfil requirements for annual certification and for appropriate
revalidation.
Annual leave entitlement is six weeks per year (pro rata for part time post holders), with additional
statutory holiday entitlement as agreed by the Local Negotiating Committee and published
annually.

The detail of fixed sessions, including any special interest, will be finalised following appointment of
the successful applicant. It is expected initially that 9 programmed activities per week will be
devoted to direct clinical care, with 1 programmed activity allocated for supporting professional
activities. Attendance at multi-disciplinary team meetings, pre-procedure consultations, or other
activities related to a particular patient’s care are included within the 9 programmed activities of
direct clinical care and will be appropriately timetabled.
Following commencement in post, depending on the needs of the service and the particular
attributes of the post holder, it may be possible to negotiate additional time for Supporting

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme
The Protecting Vulnerable Groups (PVG) scheme has replaced previously disclosure arrangements
for people who work with vulnerable groups. The duties of this post require you to undertake
regulated work with children and young people (individuals aged under 18 years) and / or protected
adults (individuals aged 16 or over who is provided with (and thus receives) a type of care, support or
welfare service).
The post holder will be required to be a member of the PVG Scheme and to consent to NHS Forth
Valley obtaining a Scheme Record or Scheme Record Update, as appropriate. Any failure to
disclose convictions could result in the withdrawal of an offer of employment.

Health Screening

This post involves the performance of exposure prone procedures and therefore the successful
applicant will be required to produce evidence of their Hepatitis B and C Immunity Status. The
successful candidate will be required to produce evidence of Hepatitis B and C status prior to
appointment and to undergo satisfactory health screening. Any offer of employment will be subject
to satisfactory Occupational Health assessment.
