Terms and Conditions:
These terms and conditions are incorporated into the agreement between the member and management of Thai Massage and
shall be applicable to all Thai Massage membership packages.
1.
Applicants are required to provide a valid identification document along with this application form (C.P.R./ Passport / Driving License).
2.
Advance booking is required for each appointment. We recommend you to check in at the reception at least 15 minutes prior to your
appointment. All efforts will be made to accommodate members arriving later than their pre-booked appointment. However, we
regret that this may not always be possible due to the pre-booked appointments of other customers. Consequently, the duration of
the massage session of late arrivals may be reduced accordingly.
3.
Cancellation of membership is permitted in exceptional cases. If the cancellation is permitted, the used session will be charged at
normal session rates (based on the type of membership) and a cancellation fee of BD. 10/- will be added. The remaining amount
will be refunded to the member. The management reserves the right to accept or reject the cancellation of membership.
4.
If the membership card is lost, BD 5/- will be charged for issuing a new membership card.

No members will be entertained without presenting an original, valid, Thai Massage membership card at the reception
before the massage session (Photocopies will not be accepted).
6.
The duration of each massage session is 50 minutes only.
7.
The Thai Massage management shall not be responsible or held liable for safekeeping, loss, theft or damage of any
member’s belongings or valuables brought in the massage session.
8.
This membership is valid for one type of massage only. This membership is not transferable. Each membership card will permit
only the member name mentioned on the card, to avail massage services.
9.
This membership will expire unconditionally on the expiry date mentioned, both in this form and on membership card. No
further extension will be granted. If the massage centre is closed on the expiry date of any membership, due to unavoidable
circumstances, then the next working day will be considered as the membership expiry date.
10. The massage is meant for short term relaxation and the relief of muscular tension ONLY and is not intended for medical treatment and
should not be relied as a substitute for professional medical advice, examination, diagnosis or treatment by a physician. This includes, but
is not limited to the following ailments, conditions and patients: Spinal stenosis, spinal disk herniation, scoliosis (Acute back pain),
Sciatica, Fractured Bones, Osteoporoses, Severe bruising, Varicose veins, Diabetic, Blood pressure, Skin allergies (Psoriasis, Atopic
Dermatitis (Eczema), Acne Vulgaris (Acute Acne) etc.), Neck and cervical disorders, History of lungs disorders (Pre-operative and Postoperative), Major and minor surgeries (Cosmetic surgeries included), Cancer, HIV/ Hepatitis, Wheel-chair borne patients, Pregnancy, etc.
11. The therapists providing the massage therapy services are not qualified to diagnose, prescribe or treat any physical or mental illness.
12. In event of any unforeseen complications to the health of the clientele, that may arise if the clientele already suffers from any of the
ailments and conditions mentioned in, but not limited to point no.10, Thai Massage, its employees, officers, management and
directors, shall not be held liable or responsible. Also the management shall not be responsible or held liable for any costs,
expenses, loss, or claims that may arise from or in relation to any medical assistance or treatment required by the member. Any or
all such costs, expenses, loss or claims shall be the total and full responsibility of the member.
13. Thai Massage management reserves the right to refuse service to any client for improper conduct. Inappropriate conduct or language
during a service will result in the immediate termination of the service. We reserve the right to define inappropriate conduct.
14. Thai Massage management reserves the right to change these terms and conditions from time to time, in its sole discretion, an d
inform the members.
15. Thai Massage management reserves the right to cancel any membership package at any time and refund the customer in
accordance with point no. 3.
