Who is eligible to tryout? Current 8th graders, freshmen, sophomores and juniors who will be students at Ursuline
Academy or Hockaday for 2014-15 and siblings/legacies of Cistercian students are eligible to try out for the Cistercian
Varsity Cheer squad.

Members of the Cistercian cheer squad should expect the following time commitments:

Practices twice a week (Mondays and Tuesdays) after school, 4:15-6:00pm at CPS.
Practices will be held the week of June 24-28th. Time/Day TBD.

Mid-August begin Monday/Tuesday practices 4:15-6:00pm.
Attend scrimmages, Thursday team dinners and football games.

PARENT CONSENT FORM
I do hereby consent and authorize ________________________________to participate
in the Cistercian Varsity Cheer program during the 2013-14 school year.
RELEASE AND WAIVER OF LIABILITY
I understand that there are certain risks associated with participation in cheerleading. I
assume all risks associated with the program and activities in which the child
participates.
It is understood and agreed that I hereby WAIVE AND RELEASE Cistercian Preparatory
School, coaches, sponsors, authorized representative(s) and school administrators from
ANY AND ALL LIABILITY or claims and damages arising from personal injury, death
or damage to property that may occur be in anyway incident to or in connection with
participation in this program, or from any NEGLIGENCE of the School, coaches,
volunteers or other representative as a result of the student/participant’s participation in
these activities.
INDEMNITY AGREEMENT
I hereby agree to DEFEND, INDEMNIFY AND HOLD HARMLESS this School, coaches,
sponsors, authorized representatives, and school administrators harmless from and
against all liability, claims and losses, damages, punitive damages, costs, expenses,
attorney’s fees, demands, suits, and causes of actions of every kind (“the claims”),
arising on account of personal injury or death or damage to property in any way incident

connection

arising
out
