Your GP is now able to send your prescriptions to
a home delivery pharmacy called Pharmacy2U.

For repeat prescriptions, you will no longer need to
request or collect them from the surgery and then take
them to the pharmacy. One telephone call to Pharmacy2U, an
email (prescriptions@pharmacy2u.co.uk) or a visit to our website
(www.pharmacy2u.co.uk) is all it takes.
• Pharmacy2U can contact you to remind you when you
need your repeat prescription and order it for you.

Introduction
This guide is to help you use our services. If you are a new patient we welcome
you to the practice.If you are already registered with us we hope this booklet
will help you make good use of our facilities.
Registration
We currently have an open list and welcome requests for registration with the
practice from patients living in or moving to the practice area (see map on back
cover). If you move out of the area, we regret that you must register with
another practice. You can register by presenting your medical card or
completing a form that is available from reception.
Patients Rights and Responsibilities
You have a right to expect a high standard of medical care from our practice
and we will try at all times to provide the best care possible within the
resources available.
In order to assist us in this we require that you take full responsibility for
ensuring that you do not abuse the service. For example, it is your responsibility
to ensure that you keep medical appointments and follow the medical advice
given.
Very occassionally a practice/patient relationship breaks down completely. In
this situation the patient may choose to register with a different practice. The
practice also has the right to remove that patient from their list. This would
generally only follow a warning that had failed to remedy the situation and we
would normally give the patient a specific reason for the removal.
Violent Patients – Zero Tolerance
The NHS operates a Zero Tolerance Policy with regard to violence and abuse
and the practice has a right to remove violent patients from the list with
immediate effect in order to safeguard practice staff, patients and other
persons.
In this situation we are obliged to notify the patient in writing of their removal
from the list and record in the patients medical records the fact of the removal
and circumstances leading to it. The PCT is then responsible for providing
further medical care for such patients.

Making An Appointment
Appointments can be made to see a Doctor on the same day you request an
appointment and can be made in person or on the telephone.
If you want to see a specific Doctor you may have to wait until they are next
in surgery.
If you do not want to be seen on the same day appointments can be prebooked, in advance, for the first hour of every week day morning surgery or the
last two appointments of the day. Remember to cancel your appointment if
you cannot make it.
Appointments can be made to see the Nurses by booking in advance.
If you would like a Chaperone present at your consultation please advise
reception when booking your appointment.
Internet Access. We also provide a service that gives patients internet access to
the practice to book, cancel or check appointments, change address details,
order repeat prescriptions,leave messages on the web. This service can also be
accessed via digital TV. To use this facility please ask one of our Receptionists
for a registration form.
The Doctor welcomes new patients to join the surgery. All newly registered
patients are offered a consultation with the Practice Nurse on joining. Medical
treatment is available from the date of acceptance.
Non NHS Requirements
For Insurance Medicals, passport applications, medicals for driving licence and
travel inoculations please contact the receptionists. Private fees apply.
Private patients are welcome.
Medical Research Council
This practice is a member of the Medical Research Council’s General Practice
Research Framework. If the practice takes part in a study your name address
and age, as recorded on our practice computer, may be passed to the Medical
Research Council. The information will remain confidential and be used only to
choose a sample of patients whom we would then contact to ask whether they
would consider helping with some of the research. You would be free to refuse
to help without your medical care being affected
Complaints Procedure
If you wish to make a complaint:  This can be made in person to the practice manager Zoe Clarke, or, if
she is not available, Dr Dean.
 Or you can make your complaint in writing to the practice manager, Zoe
Clarke.
You will receive a response to your complaint within 10 working days. If further
investigation is necessary then you will be advised of the time delay for the
response. If you are not satisfied with the outcome of your complaint to the
surgery you can contact the Chief Executive of Reading PCT or The Health
Service Ombudsman
Emergencies
In case of an emergency please telephone the surgery on 0118 9595200
During surgery hours please give the details clearly to the receptionist
emphasising that it is an emergency. Out of hours please listen carefully to the
recorded message which will inform you of the WESTCALL, on call emergency
co-operative, telephone number with whom we work.
Requests For Home Visits
Newly Registered Patients
If you are too ill to attend the surgery and need a home visit please try and
telephone before 10am. This helps the Doctor to plan his rounds efficiently. If
you feel the problem is urgent please tell the receptionists so that they can
pass you straight through to the Doctor.
Repeat Prescriptions
If you are on regular medication and your Doctor feels your medication can be
repeated without you being seen each time you will be given a repeat
prescription slip. Either hand this slip to the receptionist, fax or post it with a
stamped addressed return envelope. Please allow 2 working days for
processing. Repeat prescriptions have a review date. You need to see the
Doctor when this expires.
Telephone Advice
The Doctor are quite prepared to give telephone advice where appropriate. If
you would like to speak to the Doctor for advice then please telephone the
surgery.
Test Results
When you are telephoning for the results of tests it would assist the staff if you
could telephone between 9.30am and 4pm.
Blood tests take a minimum of 4 days
X-Rays often take 7 - 14 days
Cervical Smear Tests can take 6 weeks
Access For The Disabled
There is a purpose built entrance to the building for wheelchairs and there is a
lift to all floors. Toilet facilities are also available.
Antiseptic Solution:
One teaspoon diluted in warm water for cleaning cuts and grazes
Antiseptic Cream:
For treating septic spots, sores in the nose and grazes
Calamine Lotion:
For dabbing (not rubbing) on insect bites, stings and sunburn
Dressing Strips:
For minor cuts.
3” Wide Crepe Bandage:
To keep dressings in place. To support sprained or bruised joints.
Cotton Wool:
For cleaning cuts and grazes.
Thermometer:
For fevers.
Tweezers:
For removing splinters.
Remember that your local chemist can give you advice about medicines
The Family Medicine Chest
Here is a list of useful medicines and dressings with a description of their uses.
All are quite cheap and worth stocking at home in readiness for minor illnesses.
Keep them in a box or cupboard with a lock
or store them well out of the reach of children
Soluble Aspirin Tablets:
For adults and older children. Good for headaches,colds, sore throats and
painful bruises.
Paracetamol Mixture:
For relief of pain or fever in young children (Calpol).
Sedative Cough Linctus:
For dry or painful coughs - but not coughs caused by common colds.
Menthol Crystals:
Add to hot water to make steam inhalations for treating catarrh and dry or
painful coughs.
Vapour Rub:
Again, for steam inhalations. Also useful for children with stuffy noses or dry
coughs. Rub on the chest and nose.
Ephedrine Nose Drops:
For runny noses in children over 1 year old. Use before meals and at night but
not for more than 4 days.
Practice Staff
Practice Manager
The practice manager, Zoe Clarke, is available to help you with any
administrative or non-medical aspects of the practice. She is also available to
discuss any comments or suggestions that you may have. You can either
telephone on 0118 9595200 or, if you enquire at reception, she will be happy
to see you if she is free.
Reception Staff
We employ five part time receptionists to help us run the practice. Please
remember that they are anxious to help you but at times their job can be a
difficult one, please be patient.
Practice Nurse
You will be asked to see one of the Practice Nurses for contraception,
immunisations, ear syringing, removal of stitches, dressings and treatments for
minor accidents. They assist the Doctor in minor surgical operations and
gynaecological procedures. It is particularly recommended that women should
have regular cervical smears and that all patients should be protected against
tetanus and polio. See separate details on clinics available.
Phlebotomists
You will be asked to see one of our Phlebotomists for routine blood tests.
District Nurse
The team consists of a nursing sister who is supported by other qualified
nursing staff. They are attached to the practice and provide a nursing service in
the home to all age groups. They have knowledge of local and national services
available to patients and their families, from statutory and voluntary
organisations. If you wish to contact them, you may do so via the surgery.
Health Visitor
All Health Visitors are qualified nurses with special training and experience in
child health, health promotion and education. We have a health visitor
attached to the practice who provides mother and baby clinics. The health
visitor is also available to offer practical support and give general advice on all
health and social problems. You may contact her via the surgery or on 0118
9574844.
Midwife
contraception.
What To Do If your child has a cough that gets worse rather than better and
starts to have longer fits of coughing more and more
often, see your doctor. It is important for the sake of other children to know
whether or not it is whooping cough.
Antenatal care is the routine service provided for pregnant women.

After the birth of your baby the midwife will visit you at home. The health
visitor visits and is available for support and advise. Your 6 week postnatal
check is with your doctor who will check your weight and blood pressure, offer
a cervical smear (if necessary) and deal with any questions regarding
Measles, mumps, rubella and whooping cough can all be prevented by
immunisation.
Notes ———————
How To Recognise It Child may complain of pain around the ear or feel
uncomfortable when chewing. Swelling then starts, under the jaw up by the
ear. Swelling often starts on one side, followed
(though not always) by the other. (It is rare for mumps to affect boys testes.
This happens rather more often in adult men with mumps. However, the risk of
permanent damage is very low). What To Do Child may not feel especially ill
and may not want to be in bed. Paracetamol will ease pain in the swollen
glands. Give plenty to drink but not fruit juices. They make the saliva flow which
can hurt. No need to see the doctor unless your child has stomach ache and is
being sick.

Illness and Incubation Period 14-21 days
Infectious Period From a few days before illness starts until a week after the
rash first appears.
How To Recognise It Can be difficult to diagnose with certainty. Starts like a
mild cold. Rash appears in a day or two, first on the face, then spreading. Spots
are flat. On a light skin, they are pale pink. Glands in the back of the neck may
be swollen. Child dosen’t usually feel unwell. Give plenty to drink.
What To Do No need to see the doctor. Keep your child away from anyone you
know who is pregnant (or trying to become so). If your child was with anyone
pregnant before you knew about the illness, let them know.
Whooping Cough
Illness and Incubation Period 7-14 days
Infectious Period From the first signs of illness until about six weeks after
coughing first started, unless an antibiotic is given.
How To Recognise It It is a gradually worsening cough. After two weeks,
coughing bouts start. These are exhausting and make it
difficult to breathe. Sometimes, but not always, there's a whooping noise as the
child draws in breath after coughing. It takes some weeks before the coughing
fits start to die down.

Confidentiality
Childhood Illnesses and Symptoms
We ask for information about you so that you can receive the best possible care
and treatment. We keep this information, together with details of your care, to
ensure that your doctor or nurse has accurate and up to date information. Your
medical history will be recorded on our practice computer system which may
only be accessed by authorised members of our team.
We provide a confidential service to all our patients, including under 16’s, and
your personal health information will not be disclosed to a third party without
your prior consent.
There are times when we have to pass on information about you to other
people such as hospitals, Social Services or the Primary Care Trust. This is
always done confidentially or by removing your identifying details when they
are not essential. Everyone working for the NHS has a legal duty to maintain
the highest level of confidentiality about patient information.
If you have any worries about confidentiality or how we use your information
please speak to our Practice Manager.
The practice operates in compliance with the Data Protection
Act 1998 and the Freedom of Information Act 2000.
Chickenpox
Illness and Incubation Period 14-16 days
Infectious Period From the day before the rash appears, until the spots are dry
How To Recognise It Begins with feeling unwell, a rash and maybe a
temperature. Spots are red and become fluid-filled blisters within a day or so.
Come first on the chest and back, then spread. The spots eventually dry into
scabs, which drop off. Unless spots are badly infected, they don’t usually leave
a scar.
What To Do No need to see the Doctor unless you are unsure whether its
chickenpox or your child is very unwell and/or distressed. Give plenty to drink.
Paracetamol will help bring down a temperature. Baths, loose comfortable
clothes and calamine lotion can all ease the itchiness.
Measles
Illness and Incubation Period 7-12 days
Infectious Period From a few days before the rash appears until five days after
it goes.
How To Recognise It Rash appears after the third or fourth day of a feverish
illness. Spots are red and slightly raised; may be blotchy. Rash is not itchy. Child
becomes very unwell, with cough and high temperature. Illness usually lasts
about a week.
What To Do See your Doctor. Give rest and plenty to drink. Warm drinks will
ease the cough. Paracetamol will ease dis-comfort and lower the temperature.
Vaseline around the lips protects the skin. Wash crustiness from eyelids with
warm water.
Mumps
Illness and Incubation Period 14-21 days
Infectious Period From a few days before becoming unwell until swelling goes
down. Maybe 10 days in all.
