Eagle Point High School 2013-2014 Local Scholarship Application Return to the Eagle Center by March 7, 2014 These scholarship awards are for the academic year beginning September 2014, and are awarded for full time enrollment (12 or more credit hours) at any accredited Vocational School, Community College, Four Year College or University. All scholarships listed are contingent upon funding from donors listed. Descriptions of all scholarships include specific guidelines. While a majority of the scholarships only require a completed application packet, others require an additional essay, interview, application, activities chart or have special instructions. Additional materials required or special instructions are highlighted in bold font. Please review all scholarship descriptions carefully, apply only for those which you qualify and include all required materials. All applications must be typewritten or computer generated. You may access the application online by going to the School District 9 web page at http://www.eaglepnt.k12.or.us , select the school link at the top of the page, select Eagle Point High School, select Student Services - The Eagle Center, and click on scholarships from the tool bar on the left and follow the instructions. If you do not have access to a computer outside of school you may come to the Eagle Center and utilize one of the student computers. The Eagle Center is open before school, during lunch and after school. Please provide specific examples. 2014 Eagle Point High School Scholarships 1 ___Antonio Avila Sr. Memorial Scholarship Recipients will be graduates of Eagle Point High School whose cumulative GPA is greater than 3.00 and who are also in need of financial assistance to pursue their higher education. In addition to a completed scholarship application packet students must include a 100-300 word essay addressing "How will attending college affect the people in your life?” Essay should focus on how college affects the student's family and community. This scholarship may be renewable each year upon application. 2 ___The Atlas Group – Future Business Leaders Scholarship Recipients will be graduates of Eagle Point High School whose cumulative GPA is greater than 2.00 and who plan to pursue their higher education at a community college or four-year institution. In addition to a completed scholarship application packet students must include a 200 word essay addressing "What first motivated you to pursue a college education and how you will use it to positively impact the business community?” 3 ___Carpenter Dental Graduating seniors who are interested in pursuing a career in the dental field. 4 ___Carpenter Foundation Scholarship Criteria includes: financial need, scholarship, leadership, initiative, and character. This scholarship may be renewable each year upon application. 5 ___Cascade Wood Products Scholarship Student must major in a vocational field of study at any accredited Vocational School, Community College or University. 6 ___Charley Givan Scholarship Grant / Ben B. Cheney Foundation Preference will be given to agricultural students and students in related fields, though the scholarship is not limited to these students, and those in need of financial assistance to continue their education or training. Further preference will be given to students with ability to succeed in chosen field of study, achieve a satisfying life and contribute to the well-being of our Country. This scholarship is not restricted to those students with high achievement records and may be renewable each year upon application. 7 ___Class of ’78 Mike Bowles Honorary Endowment Recipient must be a graduate of School District #9, or successfully complete a GED or Home School Program and live within the geographic boundaries of the school district. Recipient must also attend any accredited two year, four year, or vocational/technical institute. 8 ___Complete Care Scholarship Students must have a 3.75 GPA, be involved in one extracurricular activity, and submit a 400-700 word essay on the following topic: “What is health? Why is it important? How is it achieved?” 9 ___District 9 Community Scholarship Fund A ranking of the graduating class is made based upon the criteria of 7/8 GPA and 1/8 SAT or ACT scores. The class Valedictorian(s) may receive a bonus that the District #9 Community Scholarship Committee discretion. This award is automatic based upon the above criteria, no application is necessary. 10 ___D-9 Community Service Scholarship Criteria for this award includes: GPA, community involvement and cumulative community service hours. Candidates are required to provide an activities chart, as well as a short essay highlighting their community service involvement. 11 ___D-9 Foundation Scholarship Recipient must be a graduate of School District #9, or successfully complete a GED or Home School Program and live within the geographic boundaries of the school district. 12 ___David Poole Memorial Scholarship Recipient must be an active member of EPHS FFA Chapter, EPHS Skills USA Program, or an active member of the Industrial Mechanics Program. In addition, selection may be based upon merit, classroom performance, grade point average, involvement in other school activities, and financial need. 13 ___David Woolf Memorial Scholarship Recipient is preferably a student who has had 4 years of membership in EPHS FFA Chapter. Merit, classroom performance, GPA, involvement in other activities and financial need will be considered. Recipient must be a graduate of School District #9, or successfully complete a GED or Home School program and live within the geographic boundaries of the school district. Recipient will be selected by a panel that is approved by the family of David Woolf. A 300 word essay, focusing on applicant’s plans and any other considerations in your life, past, or present is required. In addition to the essay, applicant must also include FAFSA results, GPA, SAT/ACT scores, and any additional monies (scholarships, grants, etc.) given. 14 ___Eagle Point Education Association Scholarship One scholarship will be awarded to a student who plans to major in education and pursue a teaching career. The other scholarship will be awarded, without regard to major, to a student whose parent in an EPEA Bargaining Unit Member. Student is not required to be an Eagle Point High School senior. 15 ___Eagle Point Friends of the Library Scholarship These funds are to be used towards the purchase of textbooks. There is no GPA or financial requirement for this scholarship. In addition to a completed scholarship application packet students must include a 300-500 word essay addressing “What makes the Eagle Point Library a window to the world?” Use of basic writing fundamentals is important. 16 ___Eagle Point Lions Club Scholarship All applicants will be considered; however, preference will be given to students pursuing medical career, but are not limited to. Students must submit a separate activities chart highlighting their community service, school participation, and GPA. 17 ___Eagle Point Youth Baseball Scholarship Recipient must be a member of the EPHS Baseball or Softball team and demonstrate good leadership. Selected students must also be enrolled as a full-time student at any accredited college or university AND be a participating player on the Baseball/Softball team. Students must also maintain full-time student status and eligibility requirements. This scholarship is renewable every scholastic year up to four years. 18 ___Fire District #3 Volunteer Association Scholarship Recipient will be a graduating senior of Eagle Point High School. Eagle Point High School Scholarship Committee will review applications and Fire District #3 Volunteers will review finalists. 19 ___Frank L TouVelle Trust Fund Recipient will be a male student(s) of Jackson County Oregon, is determined to be “wayward” but potentially worthy by the Touvelle Trust Committee. The primary purpose of the fund is to assist the applicant in obtaining an education and make him a better citizen. Elements such as past history and circumstances, including family situations, which may have contributed to the student’s present condition will be considered. The Touvelle Trust will consider needs other than academic. This scholarship is renewable each year upon application. A personal interview with Mrs. Snider is required for this scholarship. You must make an appointment prior to March 1st at the Eagle Center. 20 ___Harley Dickerson Memorial Fund Recipient must be a member of the EPHS Softball team. Student must be able to demonstrate motivation, good initiative, leadership, and character. Preference will be given to students with financial need and a letter of recommendation from their coach is also required. 21 ___Larry Stillman Memorial Scholarship This scholarship is available to Eagle Point High School graduates or GED recipients who will continue their education beyond high school at an accredited institution. A strong commitment and record of community service is desirable. 22 ___Local 1817 Firefighter Union Scholarship Recipient will be a graduating senior of Eagle Point High School. Eagle Point High School scholarship committee will review applications and the Local 1817 Firefighter Union will review finalists. 23 ___Robert and Carol Hayes Memorial Scholarship Two scholarships will be awarded to students with academic merit and financial need. This scholarship is renewable each year upon application. 24 ___Rogue Valley Corvettes Scholarship Student must be an active member of Skills USA. Must be enrolled or have been in a Career Technical Program at Eagle Point High School. In addition, selection may be based upon merit, classroom performance, grade point average, involvement in other school activities, and financial need. 25 ___Rogue Valley Manor Foundation Scholarship for Women (Fairy Godmother Scholarship) Recipients will be female graduates of Eagle Point High School who are in need of financial assistance to pursue their higher education. Applicant cannot be now, nor become pregnant during the term of the scholarship or they will forfeit funds. This scholarship requires an additional application which is available from Mrs. Snider. Applications for this scholarship are due by March 1, 2014 to Mrs. Snider in the Eagle Center. 26 ___Sodexo Culinary Scholarship Recipient(s) will be a graduating senior of Eagle Point High School. Recommendations will be solicited from instructor(s) for the culinary arts program at Eagle Point High School. The JCD9/Eagle Point HS scholarship committee will review applications and the Sodexo Management Team will interview finalists. In addition, selection may be based upon merit, classroom performance, grade point average, and involvement in other school Culinary/Hospitality activities. Preference will be given to students pursuing Culinary or Hospitality courses of study. 27 ___Southern Oregon Lumberman’s Association This scholarship will be awarded to student(s) whose parent(s) make a living in the wood products industry, or have become a displaced wood products industry worker. This includes mill-workers, equipment operators, loggers, retail-wholesale employees, truck drivers of wood products, etc. Financial need of qualified applicants may be considered. 28 ___Thomas Ussary Memorial Scholarship Recipients will be dedicated students who have successfully completed the Industrial Mechanics program of study at Eagle Point High School. 29 ___Tim Trautman Memorial Scholarship Student must be an active member of EPHS FFA Chapter, in addition, merit, extra-curricular activities and GPA will be taken into consideration.
