NW Thames Foundation School Individual Placement Description Placement F1 Paediatric Surgery with General Paediatrics on Paediatric Short Stay Unit (2 months) The department The department of Paediatric Surgery at St Mary’s Hospital currently comprises a SASG Paediatric Surgeon, a registrar (changes daily) and the F1. Paediatric Surgeon including consultants and registrars from Chelsea and Westminster Hospital run weekly outpatient clinics in the department and also have operating lists that take place on this site. Paediatric Short Stay Unit is the base for short stay cases referred from A&E and for Great Western Ward day case follow up. This is the area in which we are developing our integrated Paediatric work pathways. Including hospital at home, rapid referral clinics, email and phone advice service supported by its own team of Registar, F2, F1 and nurses. Learning opportunities include: Weekly protected 2 hours teaching time on Thursdays and registrar led resus teaching every Friday morning Consultant review of any surgical patients Opportunities to examine and formulate management plans Paediatric prescribing skills Manage time and clinical priorities effectively Communicate effectively with children and their parents/relatives Opportunity to assess and review patients independently in the pre-op assessment clinic Opportunity to lead the prolonged jaundice babies clinic - from diagnosis to management, and follow up Opportunity to develop surgical skills by attending theatre Use evidence, guidelines and audit to benefit patient care Community experience offered in this placement includes: Clinical Supervisor(s) placement for Main duties of the placement Close liaison with community services around complex discharges and ongoing community care Weekly MDT to discuss complex patients moving between primary secondary and tertiary care Opportunity to follow patients home with AHPs for follow up experience post discharge Opportunity for community visits with AHPs and community Paediatricians Working within innovative hospital to home service the Dr Tagore Charles, Consultant Paediatrician The F1s main duties include: Leading the prolonged jaundice clinic for babies, assisted by the consultant. Running the Paediatric Surgery pre-op assessment clinic on Tuesday and Friday morning, daily review of any surgical inpatients with the consultant and completing jobs for these patients if needed. The Paediatric Surgical F1 is required to complete the Paediatric surgical EDCs for patients on Westway who have come in for day case procedures. Other duties include arranging for patients to be put on the emergency theatre lists. Any other time should be spent on the Paediatric Short Stay Unit (PSSU) assisting the team managing the patients. There is opportunity to join the team in the Monday outpatient clinic and to go to theatre. Along with the F1 in General Paediatrics, the Surgical F1 should attend the chronic patient meeting on Thursday, all teaching sessions, educational meetings and formal handover which takes place every morning and evening.
