Please alert the provider that the patient may need to see a clinical pharmacist if one or more of
the following issues occurs.
Patient issues











experienced an adverse drug event or potential adverse drug event that was not
previously addressed
is on 5 or more medications
has more than one provider
was recently hospitalized
was recently in the ER for a medication-related issue
has experienced any other care transitions
has diabetes
is being treated for hepatitis C
has asthma and is currently sick with a cold
is taking warfarin
is on a current medication therapy is not effective
