Dear Lacrosse Players and Families,
Black Dog Recreation is pleased to offer a girl’s high school and middle school lacrosse league
for girls in grades 7-12. The league will consist of officiated 8 v 8 games between school
teams. The schedule will be determined after all teams have registered. One hour games will
be scheduled on Mondays from 5-6 or from 6-7pm at the MultiPlex (Word Church) , 18909
South Miles Road, Warrensville Heights., OH, 44128. Additional information can be found at

Mondays: Nov. 7th, 14th, 21st, and 28th; Dec. 5th and 12th; Jan. 2nd, 9th,
16th, 23rd and 30th; Feb. 6th, 13th and 20th
Time:
Mondays: 5:00-6:00 or 6:00-7:00 Team schedule TBA
Cost: $1,700.00 per team (Individual players $150/player)

that by my participation in Black Dog Recreation LLC events, I assume all risks which may arise from such participation including,
without limitation, catastrophic injury, paralysis, and even death. On behalf of myself and my heirs, executors, representatives and
assigns, I hereby release, discharge and exonerate Black Dog Recreation LLC and its trustees, directors, officers, employees, agents,
volunteers and coaches from any and all liability for damages of every nature, kind, and description including, without limitation,
injury, loss of life or other loss or damage occurring as a result of my participation in Black Dog Recreation LLC events.
