Further notes can be attached on separate sheets. Note that evidence from the Supporting Documents should also be included where applicable to support the overall assessment. This report form (CGeol ASS3) is designed for you to provide information for the Chartership Officer and the Chairman of the Chartership Committee in order that they may give, where necessary, the Applicants, their Mentors and their Sponsors the reasons for your recommendation. The report also acts as an audit trail demonstrating the value and rigour of the CGeol Validation process. For very good Applicants, who readily fulfil all of the Chartership criteria, little needs to be written – although positive and, or, complimentary comments are always welcome. For those Applicants that do not reach the required standards, in any or all criteria, we ask that you provide an explanation for your decisions and advice for a future application. These comments on deferred Applicants are also of value when the recommendation is reviewed by two members of the Review Panel. Applicants that only just reach the necessary standards will also benefit from advice on how they may develop and improve their competences further as there is commonly little difference between those who just miss out and those that just make it. In exceptional circumstances the Scrutineers may record a decision of Accept (conditional on submission of additional evidence). This option is available only if the Scrutineers consider that the Applicant has the levels of competence required to fulfil each criterion, but has narrowly failed to support this sufficiently through their written application. It is a requirement that this evidence would already exist (eg, technical papers or reports) or may be easily generated (eg, CPD records). The Applicant will be required to produce the evidence to be considered by the Scrutineers within 4 weeks, when they will make a final recommendation to Accept or Defer. If ‘Acceptance conditional on’ is recommended please give your reasons for this and list the additional material required in the Feedback box on page 5. You are asked to make comments against each of the seven competence criteria using the abbreviations in the following list to indicate their level of achievement. This list is a progressive sequence with Ap as the lowest level and C as the Acceptance level.
