Job Description Service CRI Street Outreach Team Job Title Street Outreach Worker (Generic) Hours 37.5 hours Salary Range £21,356 - £25,065 per annum Base Cambridge Reports to Project Manager Purpose of Job The Street Outreach Team is made up of generic street outreach workers and mental health outreach workers and the full range of services is provided by all workers across Cambridge City to reduce rough sleeping and reduce street based anti-social behaviour. In addition, the team is responsible for identifying and supporting rough sleepers or those at risk of homelessness who have diagnosed or undiagnosed mental health support needs. The post holder is responsible for providing the full range of services across Cambridge City in an outreach and in-reach capacity as well as at the homes of clients where appropriate. The post holder will carry a case load and will be responsible for carrying out risk assessments, comprehensive assessments, recovery plans, recovery plan reviews and on going case management for all clients. The post-holder will contribute to the service to ensure that rough sleepers and those at risk of homelessness consistently receive high quality services that adhere to best practice guidelines and achieve high professional standards. The post holder will be required to work flexibly across operational sites as required and within an agreed number of hours to maintain the most appropriate level of service provision. This will include evening working. December 2010 The post holder will be expected to take responsibility for personal development, identifying personal training needs and participate in regular supervision and appraisal. Key Activities: Identify, engage and support people who are rough sleeping, engaged in street based anti-social behaviour, at risk of homelessness or those who present with Mental Health support needs. Carry out risk and comprehensive assessments and develop individual recovery support plans with all clients. Utilise motivational interviewing and brief interventions in one-to-one settings to promote engagement in treatment and support services to encourage self-esteem, well-being, self-responsibility and enhanced motivation. Manage a client caseload and maintain case management responsibility for all these clients throughout their time with the Street Outreach Team. Review on going support through liaising closely with all members of the Street Outreach Team and other partnership forums where appropriate. Provide a full range of services in an outreach and in-reach capacity including home visits where appropriate across Cambridge City. Work and liaise with other agencies involved with the client’s broader care plan including housing and accommodation providers, mental health services, hospitals and GP’s / primary care. Engage in regular clinical supervision where appropriate and maintain a current knowledge of appropriate interventions with regard to clients presenting with Mental Health support needs. Actively participate in regular management supervision and be willing to personally reflect on interpersonal dynamics within the therapeutic relationship and provide evidence of achieving performance targets alongside other supervision requirements as detailed in CRI policy. Work within CRI’s professional development framework participating in active learning and developing professionally, in accordance with service and organisational requirements and current skills and abilities. Recognise personal and professional boundaries and work within CRI governance framework at all times. Present a professional image and actively promote the service at all times to service users, other stakeholders, partnership organisations and the general public. Act as the lead Street Outreach Worker within the team in safeguarding, substance misuse and dual diagnosis, relocations and chronic exclusions or sex workers. Participate in and make use of supervision and appraisal in line with CRI policies. Experience of working within housing or homelessness services. Proven understanding of the need for and ability to deliver quality services. Experience of identifying and supporting those who present with mental health concerns. Working knowledge of mental health, substance misuse and complex needs. Experience of tackling street based activity including anti social behaviour. Good working knowledge of the Criminal Justice System. Understanding and experience of the effective use of Brief Interventions. Demonstrable partnership working and relationship management skills. Experience of meeting demanding targets and deadlines.
