Administration of oral or transdermal medications is provided at $3.25 a day with owner
provided medications.
Administration of insulin injections is provided for $4.25 per injection with owner provided
medication and syringes.
Administration of subcutaneous fluids is provided for $23.50 per day with owner provided
fluids, needles, and IV line.

Requirements/Release for Boarders
1. All cats must be current on Distemper/upper respiratory and rabies vaccines and the owner must
provide written proof or telephone confirmation from a veterinarian at time of check in, without
which the cat(s) will be examined and vaccinated at owner’s expense.
2. All cats must be free from external parasites and cats found to have evidence of parasites will be
treated at the owner’s expense.
3. In the case of illness or injury, the proper attempt will be made to contact the owner or
representative. In the event we are unable to establish contact, I, the undersigned, do hereby give
my consent for the Doctors of the Ann Arbor Cat Clinic to perform necessary treatment upon my
cat(s) while they are being boarded.
4. All responsible precautions will be used against illness, injury, or escape of my cat(s), but the Ann
Arbor Cat Clinic will not be held liable or responsible in any manner, under any circumstances, on
account of the care, treatment, or safe keeping of my cat(s), as it is understood I assume all risks.
5. Should circumstances arise that my cat(s) remained unclaimed after the date that I have stated
as the pick up date, I understand that written notice will be mailed to my current address. Seven
days after such written notice, the cat(s) will be considered abandoned and may be disposed of, or
destroyed, as the Ann Arbor Cat Clinic deems best. It is further understood that such action will not
relieve me from my financial responsibility for all costs of services provided, including cost of
boarding services.
