Attachment 1 Achievement and Accountability Office Knowledge Management Department - Student Records Checklist of Documentation to Accompany Student Cumulative Records Folders When Student Is Transferring to Another Public School in the District or in Maryland Transfer of Student Cumulative Record Folders to Another Public School in the District: The following items are to be provided to the RECEIVING school by the SENDING school: A list of students whose records are being forwarded (copy retained at SENDING school); A list of students attending summer school; o Records of students attending summer school will be forwarded as soon as possible after summer school ends (August 2015). For each student transferring: o Cumulative Record Folder (including complete report cards in the cumulative record folder) o Original Section 504 Plan Records (placed behind the cumulative record) o Original Discipline Records (placed behind the cumulative record) A signed verification of records accuracy and completeness by staff preparing the records for transfer. Note: The sending school should maintain copies of the following for four (4) years: All SR Cards Health Information Current Individualized Education Program (IEP) or Individualized Family Service Plan (IFSP) Most recent assessment reports Documentation of the disability for students with disabilities 1 Attachment 1 Transfer of Student Cumulative Records Folders to a Public School in Maryland The following items are to be provided to the RECEIVING school by the SENDING school upon receipt of an “official” request for records from a sending school: Provide a SR 7 card to the parent requesting the transfer o If parent is not present, then the SR7 card must be faxed, sent electronically, or mailed by the Sending School. Send the original student records in their entirety Special education records Discipline records Note: The sending school should maintain a copy of all sides of SR 1,2,3, as well as a copy of the documentation of the disability for students with disabilities who have received/are receiving special education services. 2
