Program Letter of Agreement between the University of Arkansas for Medical Sciences Area Health Education Centers (AHEC) Family Medicine Residency Training Program and Joseph Dunaway, DO I. Purpose: This document serves as an Agreement between the AHEC – South Central, Family Medicine Residency Training Program and Joseph Dunaway, DO involved in resident education. This Letter of Agreement is effective from March 26, 2013 and will remain in effect for five (5) years, or until updated, changed or terminated by the AHEC – South Central and/or Dr. Dunaway. II. Officials Responsible for Education and Supervision: The resident training program is administered by the Program Director, Toni Middleton, MD. The Program Director is an employee of AHEC, a member of the University of Arkansas for Medical Sciences College of Medicine faculty, and has the overall authority and responsibility for the residency program. The Program Director assures that the general quality of the residency program is consistent with guidelines established by the ACGME. The Program Director appoints the residents to the program, selects the teaching staff, and determines the rotations and assignments of both residents and members of the teaching staff. During rotations with Mr. Morse/Johnson Regional Medical Center, the attending physician – Dr. Joseph Dunaway is responsible for the education and supervision of the residents assigned to his/ specific service. V. Responsibilities: The faculty attending, Dr. Joseph Dunaway, at Johnson Regional Medical Center will provide appropriate supervision of residents in patient care activities and maintain a learning environment conducive to educating the residents in the ACGME competency areas of patient care, medical knowledge, practice-based learning and improvement, professionalism, interpersonal and communication skills, and systems-based practice. The faculty will evaluate resident performance in a timely manner during each rotation or similar educational assignment and document this evaluation at the completion of the assignment. III. Content and Duration of the Educational Experience: The goals and objectives for the educational experiences of the residents will be consistent with those of the ACGME Family Medicine Residency Program Requirements. The goals and objectives for a specific rotation and the resident’s responsibilities will be reviewed with the resident by the attending physician at the beginning of the rotation. The content of the educational experience will adhere to that described in the rotation curriculum. The duration of the assignment to Dr. Dunaway at Johnson Regional Medical Center is four weeks. IV. Policies and Procedures: During rotations with Dr. Dunaway at Johnson Regional Medical Center, the resident is obligated to comply with all policies and procedures of the practice.
