HOYT STREET ASSOCIATION COMMUNITY FUNDING PROJECT GRANT APPLICATION 2015 - 2016 HSACFP MISSION STATEMENT: To preserve and enhance the Boerum Hill neighborhood and to encourage the maintenance of all public facilities and services within the Boerum Hill community. To create and maintain one or more community parks and gardens for the enjoyment and use of community residents and the general public. To foster a community of good neighbors in the Boerum Hill neighborhood and surrounding area by lessening neighborhood tensions, prejudice and discrimination; and To work toward the improved safety and security of the Boerum Hill neighborhood by combating crimes against persons or property in the neighborhood and the surrounding area. HSACFP GRANT AREA: The boundaries of the area to be served by the grant shall be: Schermerhorn Street on the North Warren Street on the South Fourth Avenue on the East Court Street on the West HSACFP PROPOSAL STIPULATIONS: Proposals should meet one or more of the following needs within the grant area stated above. - Encourage cleanliness, orderliness, beautification or preservation - Improve public safety or quality of life - Support educational or cultural activities for the residents - Promote harmony between diverse elements of the neighborhood HSACFP PROPOSAL EVALUATION: - Grants are intended to support projects of modest scope with seed money. The proposal should provide information about the volunteer participants and additional funding from other sources. - The Evaluation Committee will favor proposals with ongoing impact beyond the life of the grant and reasonable prospects for future funding. - The Evaluation Committee will favor proposals for equipment and supplies other than those normally used for day-to-day operations. HSACFP GRANT AMOUNTS: - A single grant will not exceed one thousand dollars. - The Evaluation Committee will not be required to expend all budgeted funding available in a calendar year. Budgeted funds not given in grants will return to the general fund of the Hoyt Street Association. - The Evaluation Committee may fund a grant proposal at less than the requested amount. - The Evaluation Committee may not fund a grant proposal at more than the requested amount. - At the end of the grant year, grant funds not used by the grantee are the property of the Hoyt Street Association. INTRODUCTION AND/OR STATEMENT OF PURPOSE: Who are you? (What is your name and if you are a part of an organization, what is the name of your organization’s name, address and phone number? Who are the key persons to contact?) - What do you or the organization want to accomplish? -Where do you or the organization want to do the project? - Are there any special legal or other issues (permits, etc.) attached to the project? Have you addressed the relevant agencies? Provide evidence. 2. NEED: State the need to be filled or the problem to be solved. 3. PLAN: State briefly how the plan or project addresses the NEED stated above. 4. EVALUATION: State briefly how the success of the plan or project’s implementation will be evaluated. 5. BUDGET: List the relevant costs for equipment and personnel. If the useful life of the equipment purchased will exceed the life of the project, provide information as to the deployment of the equipment. 6. FUTURE FUNDING: If your project is ongoing beyond the calendar year of the grant, how do you plan to fund it in the future? 7. QUALIFICATIONS and DOCUMENTATION: Explain how the people or organization(s) carrying out the project have the ability to tackle the problem in question. Provide any other information that will help the committee reviewing this grant request. HOYT STREET ASSOCIATION COMMUNITY FUNDING PROJECT GRANT CALENDAR 2015-2016 Applications must be postmarked or delivered by 5:00 PM on October 2, 2015 Applications will be reviewed from October 2, 2015 - October 30, 2015 Grant(s) will be awarded in November 2015 (Since the Hoyt Street Association may not be able to fund all who apply, we urge those who do not receive a grant to reapply again next year. Terms for receiving HSACF grants: 1. No person or organization may receive a grant if that person or organization was funded in the immediate prior grant cycle. 2. Generally, the maximum amount of any single grant is $500.00. Amounts up to $1000.00 may be granted under exceptional circumstances. It is understood that the money granted will be used only on the project as defined in the application and will be expended within one year of the grant’s receipt. If not, unused funds must be returned to the Hoyt Street Association. 3. Please save a copy of your completed application for your files. Each person or organization receiving a grant will be required to provide a report at the end of the project or calendar year, whichever comes first, to the Hoyt Street Association. This report must account for how the money was spent. (You are required to provide receipts for major expenditures of $100 and up.) Include an evaluation of the success of the project.
