Melton Shire Council 2013 Community Achievement Awards As part of the annual Australia Day celebrations, Melton Shire Council recognises local groups and individuals who have made an outstanding contribution to the Melton Shire community through the Community Achievement Awards. Nominations are sought for: Citizen of the Year Youth Citizen of the Year (under 25) Community Group / Project of the Year Nominations can be submitted by completing the attached form. When answering the questions, the responses should be based on meeting the selection and eligibility criteria. If you require assistance completing the form, contact Council’s Events and Culture Coordinator on 9747 7200. When submitting an application, the use of supporting documentation is encouraged; this can include personal references, portfolios, or details of work etc. Please provide photocopies of any supporting documentation that you provide, as Melton Shire Council will keep all the information sent with applications. Community Achievement Awards Selection Criteria An independent panel will review all nominations, looking for nominees that reflect the values of the award as outlined below: Commitment to improving community access, participation and involvement. Contribution to the Melton Shire community. Fostering community partnerships and connections. Demonstration of community leadership. Eligibility Citizen Award nominees must be Melton Shire residents. Community groups and projects must be based in the Shire of Melton. Projects nominated for Community Project of the Year must have been completed between the 1st January 2012 and 23rd November 2012. Contributions made by nominee, must have either been done on voluntary basis or exceed the normal requirements of the person’s paid work. Recipients of the Awards (or proxy) must attend the Australia Day presentation of these Awards. Unsuccessful nominees can re-nominate in subsequent years. Citizen of the Year and Youth Citizen of the Year (under 25): This award recognises an individual who has made an outstanding contribution to the Melton Shire community, making it a better place to live. The Selection Panel will use the following criteria to select the Citizen of the Year: Demonstrated contribution to the community. Involvement in activities, which promote community welfare and well being / have community benefits. Provides an inspirational positive role model and/or shows leadership skills. Melton Shire Council 2013 Community Achievement Awards Community Group / Community Project of the Year: Awarded to a community group (group consisting of four or more people) recognising the value of their service to the community or to a program or project (inc capital works) that has benefits for the wider community. The project should demonstrate innovation and excellence with a high level of public participation. Community Group / Project of the Year will be selected on the following criteria: Who was involved in the project / group and what outcomes were achieved? How does the group / project make a difference in the community? What service or benefits does the group /project provide to the community? How does the group / project reflect the values (selection criteria) of the Community Achievement Awards? Announcements Award winners will be contacted mid December 2012 with Awards officially presented to recipients at the Australia Day Celebrations on Saturday 26th January 2013. Recipients (or proxy) must attend these awards to be eligible. All nominations will be acknowledged at the Australia Day Celebrations. Winning nominations details will be used for publicity purposes. All supporting documentation will be kept confidential and used to assist the Selection Panel to consider the merits of nominations.
