Step 2: Summarize Sheri Sangji’s story in the space below. Be sure to (a) summarize the factors that contributed to this

Sheri died in a lab accident caused by the spontaneous combustion of a highly reactive chemical. This spontaneous
reaction caused her flammable shirt to catch on fire. This fire was aggravated because Sheri had other flammable
chemicals in the hood at the same time and she knocked it over. There was not appropriate body protection utilized by
Sheri or offered by her lab. If Sheri had a flame resistant lab coat, that may have helped provide her with more time to
react. UCLA failed to appropriately train their employees in lab safety. UCLA is improving safety accountability and
oversight, improving training, providing proper protective equipment, conducting unannounced safety inspections, and
improving laboratory safety culture, to try to avoid future incidents like this.
