Ⅰ. Preface
GUOC will provide an efficient and effective pharmacy service for the FISU
Family and NUSF(National University Sports Federation) Teams during the 28th
Universiade Gwangju 2015.
This Pharmacy Guide, approved by the FISU Medical Committee (FISU CM)
Chairman, contains the definitive list and prescription procedures of medications
available from the athletes' village polyclinic pharmacy, basic drug information
about each medication and information on the status of the drugs, such as
substances that are prohibited by the World Anti-Doping (WADA) Code and
special requirements of the FISU CM.
The Athletes Village polyclinic is located in the International Zone and it is open
from 9am to 9pm, with 24-hour emergency services. Its pharmacy offers a range
of medications, at free of charge, for the treatment of immediate and/or acute
medical conditions.
However, each team is advised to bring sufficient quantities of necessary /
prescribable items, such as dressings and sterilized gauze for individuals with a
chronic condition, in order for the usage during their stay in Korea.
Medications will only be dispensed from the Athletes Village polyclinic
pharmacy upon the submission of a correctly completed official Universiade
clinic prescription form which will be reviewed by a polyclinic doctor. Prescribed
Medications should be prescribed for specific individuals, in quantities required
for immediate and necessary use.
Upon necessity, the polyclinic manager reserves the right to review each
prescription, which is prepared by NUSF team doctors. The pharmacy does not
provide the medications which are not included in the pharmacy list.

Special procedures are in place for the prescription of medications classified as
"Prohibited" to athletes. The Prohibited List 2015 of WADA, an International
Standard of the Code, will be applicable and enforced during the 28th Universiade
Gwangju 2015. Doctors writing prescriptions, as well as the athletes for whom
the prescriptions are written, should acquaint themselves with the WADA Code,
particularly the WADA’s Prohibited List 2015. Special attention is needed for the
prescription of medications classified as "Prohibited"
It is the responsibility of the athletes to determine whether a substance they are
using, or considering using for medical purposes, is prohibited.
Before and during the games, athletes are strongly advised to check the status of
the medication they are taking or intend to take through the following websites:

The temporary registration of NUSF team doctors for the period of the 28th
Universiade Gwangju 2015 is restricted to caring for members of their own team
or to members of other teams where there is a prior agreement.
The medical program will provide comprehensive, convenient and effective
health care services to all participants of the 28th Universiade Gwangju 2015.

Ⅱ. Information about Prescriptions
1. General Guidelines
All medications will be dispensed by prescription only.
There are electronic prescriptions and handwritten prescriptions.
Electronic prescriptions will be prescribed by the polyclinic doctors for patients,
while handwritten prescriptions will be prescribed by registered NUSF team
doctors.
Dedicated handwritten prescription pads will be available at the pharmacy of the
Athletes Village polyclinic. The pharmacy will not provide medication according
to the prescription handwritten by a team doctor. Once the prescription has been
handwritten and submitted, a polyclinic doctor will electronically process it for
submission to the pharmacy where the medication can be received after checking
the receiver’s identification through their AD card. Both handwritten and
electronic prescription will be stored in the Athletes Village pharmacy.
A prescription for an outpatient can be submitted once it has been fully completed
according to the outpatient’s medical exam results. In case a disagreement occurs
between the team doctor and the polyclinic doctor regarding prescribed details,
the decision of the polyclinic shall be followed.
All prescriptions written during the Universiade Gwangju 2015 must be
completed clearly in Korean or English according to the form. The use of Latin
and abbreviations will not be accepted.
Please write instructions in a manner such as, "20mg, three times daily", and
avoid Latin or written abbreviations on the prescriptions.

Each prescription must be written completely, including;
- Venue and Date
- Patient's name, accreditation number and NUSF
- Gender and age
- Diagnosis
- Name of medicine
- Daily dosage, total days for use, directions for use
- "Prohibited" substance or not
- Doctor's name and signature
Athlete's signature is required when prescribing "Prohibited" substances
The prescribing doctor's name (clearly printed) and signature are required for all
prescriptions. Prescriptions can be written for up to three days (or five days for
antibiotics). And doctors have the right not to repeat the prescription according
to the patient's condition, before the last prescription is used up.
2. Prescribing "Prohibited" substances
The pharmacy of the Athletes Village polyclinic will stock a limited number of
prohibited medications. These will be flagged as such on the pharmacy computer
system and clearly branded at the time of dispensing. The status of these
medications is identified in this guide, next to the medication.
Intravenous infusions and/or injections of more than 50mL per 6-hour period are
prohibited except for those legitimately received in the course of hospital
admissions or clinical investigations.
All substances are classified as;
Permitted substances: These are drugs and methods that are neither prohibited
nor subject to any restrictions according to the WADA Code
Prohibited substances: These are drugs and methods that, according to the
WADA’s Prohibited List 2015, cannot be used by athletes who will be competing

in the Universiade unless they have a valid TUE(Therapeutic Use Exemption) for
the substance.
2.1. Prescribing Substances that have received approval from the FISU
Therapeutic Use Exemption Sub Committee (FISU TUEC)
If an athlete attending the Universiade Gwangju 2015 requires the "Prohibited"
Substances, he/she should apply TUE to the FISU CMI or TUEC following the
procedures to obtain TUE.
Procedures
① The Prescribing doctor must confirm that the athlete has granted approval
of a TUE by the FISU TUEC;
② The Prescribing doctor must explain to the athlete that a "Prohibited"
Substances is being prescribed and the possible consequences of this
action.
③ The Prescribing doctor and the athlete (if conscious and able to do so)
receiving the medication will be asked to sign and print his/her name on
the prescription to confirm that he/she is aware of the medication's status.
④ The pharmacists will confirm with the prescribing doctor that the athlete
has granted approval of a TUE by the FISU TUEC. The pharmacists will
stamp the prescription "Approval Use".
⑤ The medication is dispensed when the pharmacists confirm the accuracy
between the TUE application Form and prescribing information. The
medication will be clearly branded "Prohibited". The copy of the TUE
application Form and TUEC Decision Form will be kept in the pharmacy
of the athletes' village polyclinic.

2.2. Prescribing Substances that haven't received approval from the FISU
TUEC
The use of prohibited substances, indicated on WADA’s Prohibited List 2015,
should be avoided for athletes except during a medical emergency in which there
is no alternate to using a "Prohibited" substance. If the prohibited medication is
to be administered in a venue, the venue medical officer must notify the GUOC
medical officer who will notify the Chairman of the FISU TUEC.
If the medication is to be administered and dispensed at a designated hospital, the
administering doctor/pharmacist shall notify the clinic Manager who, in turn, will
notify the GUOC medical officer who is responsible of notifying the Chairman
of FISU TUEC.
If an NUSF administers or prescribes the medication, they will be responsible for
notifying the Chairman of the FISU TUEC. In all cases, the NUSF will be
responsible for initiating a TUE application to the FISU TUEC.
The form must be completed as soon as possible after the incident and forwarded
to the Doping Control Center in the Athletes Village. The Doping Control Center
will forward the completed TUE application form to the FISU TUEC or FISU
CM.
Procedures
① The prescribing doctor must explain to the athlete that a "Prohibited"
substance is being prescribed explain the possible consequences of this
action.
② The prescribing doctor and the athlete (if conscious and able to do so)
receiving the medication will be asked to sign and print his/her name on
the prescription to confirm that he/she is aware of the medication's status.
③ The pharmacists will confirm with the prescribing doctor that they have
prescribed a "Prohibited" substance.

④ The pharmacists will stamp prescription "Prohibited" and the medication
will be branded "Prohibited" clearly.
⑤ Either the NUSF prescribing doctor or the clinic manager must notify the
FISU TUEC that a "Prohibited" substance has been dispensed /
administered to an athlete.

Ⅲ. Drug Formulary
The drugs included in the Drug Formulary Guide are arranged according to
pharmacological/therapeutic classification.
The following prescribing information is included;
Drug classification
- Generic drug name
- Brand: It depends on the pharmacy circumstances; the brand name of
medications may vary change within same substances.
- Available form: Strength and formulation
- Major indications; Common indications are included, although the drug may
be used for other medically justified purposes.
- Dosage: The Usual recommended dose is included for a health adult with
normal renal and live function.
- Side effects: The most common side effects are listed for each drug.
- Status: All substances are classified as Permitted and Prohibited.

In accordance with Article 4.2.2 of the World Anti-Doping Code, all Prohibited Substances shall be
considered as “Specified Substances” except Substances in classes S1, S2, S4.4, S4.5, S6.a, and Prohibited
Methods M1, M2 and M3.

S2. PEPTIDE HORMONES, GROWTH FACTORS, RELATED SUBSTANCES AND MIMETICS
The following substances, and other substances with similar chemical structure or
similar biological effect(s), are prohibited:
1. Erythropoietin-Receptor agonists:
1.1
Erythropoiesis-Stimulating Agents (ESAs) including e.g. darbepoietin
(dEPO); erythropoietins (EPO); EPO-Fc; EPO-mimetic peptides (EMP), e.g.
CNTO 530 and peginesatide; and methoxy polyethylene glycol-epoetin beta
(CERA);
1.2
Non-erythropoietic EPO-Receptor agonists, e.g. ARA-290, asialo EPO and
carbamylated EPO;
2. Hypoxia-inducible factor (HIF) stabilizers, e.g. cobalt and FG-4592; and
HIF activators, e.g. argon, xenon;
3. Chorionic Gonadotrophin (CG) and Luteinizing Hormone (LH) and their releasing
factors, e.g. buserelin, gonadorelin and triptorelin, in males;
4. Corticotrophins and their releasing factors, e.g corticorelin;
5. Growth Hormone (GH) and its releasing factors including Growth Hormone Releasing
Hormone (GHRH) and its analogues, e.g. CJC-1295, sermorelin and tesamorelin; Growth
Hormone Secretagogues (GHS), e.g. ghrelin and ghrelin mimetics, e.g. anamorelin and
ipamorelin; and GH-Releasing Peptides (GHRPs), e.g. alexamorelin, GHRP-6, hexarelin
and pralmorelin (GHRP-2).
Additional prohibited growth factors:
Fibroblast Growth Factors (FGFs); Hepatocyte Growth Factor (HGF); Insulin-like
Growth Factor-1 (IGF-1) and its analogues; Mechano Growth Factors (MGFs);
Platelet-Derived Growth Factor (PDGF); Vascular-Endothelial Growth Factor (VEGF)

and any other growth factor affecting muscle, tendon or ligament protein
synthesis/degradation, vascularisation, energy utilization, regenerative capacity or
fibre type switching.
S3. BETA-2 AGONISTS
All beta-2 agonists, including all optical isomers, e.g. d- and l- where relevant, are
prohibited.
Except:
 Inhaled salbutamol (maximum 1600 micrograms over 24 hours);
 Inhaled formoterol (maximum delivered dose 54 micrograms over 24 hours);
and
 Inhaled salmeterol in accordance with the manufacturers’ recommended
therapeutic regimen.
The presence in urine of salbutamol in excess of 1000 ng/mL or formoterol in excess of
40 ng/mL is presumed not to be an intended therapeutic use of the substance and will
be considered as an Adverse Analytical Finding (AAF) unless the Athlete proves,
through a controlled pharmacokinetic study, that the abnormal result was the
consequence of the use of the therapeutic inhaled dose up to the maximum indicated
above.
S4. HORMONE AND METABOLIC MODULATORS
The following hormones and metabolic modulators are prohibited:
1. Aromatase inhibitors including, but not limited to: aminoglutethimide; anastrozole;
androsta-1,4,6-triene-3,17-dione (androstatrienedione); 4-androstene-3,6,17 trione
(6-oxo); exemestane; formestane; letrozole and testolactone.
2. Selective estrogen receptor modulators (SERMs) including, but not limited to:
raloxifene; tamoxifen and toremifene.
3. Other anti-estrogenic substances including, but not limited to: clomiphene;
cyclofenil and fulvestrant.
4. Agents modifying myostatin function(s) including, but not limited, to: myostatin
inhibitors.

5. Metabolic modulators:
5.1 Activators of the AMP-activated protein kinase (AMPK), e.g. AICAR; and Peroxisome
Proliferator Activated Receptor δ (PPARδ) agonists, e.g. GW 1516;
5.2 Insulins;
5.3 Trimetazidine.
S5. DIURETICS AND MASKING AGENTS
The following diuretics and masking agents are prohibited, as are other substances with
a similar chemical structure or similar biological effect(s).
Including, but not limited to:

Desmopressin; probenecid; plasma expanders, e.g. glycerol and intravenous
administration of albumin, dextran, hydroxyethyl starch and mannitol.

Acetazolamide; amiloride; bumetanide; canrenone; chlortalidone; etacrynic acid;
furosemide; indapamide; metolazone; spironolactone; thiazides, e.g.
bendroflumethiazide, chlorothiazide and hydrochlorothiazide; triamterene and
vaptans, e.g. tolvaptan.
Except:
 Drospirenone; pamabrom; and topical dorzolamide and brinzolamide.
 Local administration of felypressin in dental anaesthesia.
The detection in an Athlete’s Sample at all times or In-Competition, as applicable, of any
quantity of the following substances subject to threshold limits: formoterol, salbutamol,
cathine, ephedrine, methylephedrine and pseudoephedrine, in conjunction with a
diuretic or masking agent, will be considered as an Adverse Analytical Finding unless the
Athlete has an approved TUE for that substance in addition to the one granted for the
diuretic or masking agent.

PROHIBITED METHODS
M1. MANIPULATION OF BLOOD AND BLOOD COMPONENTS
The following are prohibited:
1. The Administration or reintroduction of any quantity of autologous, allogenic
(homologous) or heterologous blood, or red blood cell products of any origin into
the circulatory system.
2. Artificially enhancing the uptake, transport or delivery of oxygen.
Including, but not limited to:
Perfluorochemicals; efaproxiral (RSR13) and modified haemoglobin products, e.g.
haemoglobin-based blood substitutes and microencapsulated haemoglobin
products, excluding supplemental oxygen.
3. Any form of intravascular manipulation of the blood or blood components by
physical or chemical means.
M2. CHEMICAL AND PHYSICAL MANIPULATION
The following are prohibited:
1. Tampering, or Attempting to Tamper, to alter the integrity and validity of Samples
collected during Doping Control.
Including, but not limited to:
Urine substitution and/or adulteration, e.g. proteases.
2. Intravenous infusions and/or injections of more than 50 mL per 6 hour period except
for those legitimately received in the course of hospital admissions, surgical
procedures or clinical investigations.
M3. GENE DOPING
The following, with the potential to enhance sport performance, are prohibited:
1. The transfer of polymers of nucleic acids or nucleic acid analogues;
2. The use of normal or genetically modified cells.

section of the 2015 Prohibited List were already considered prohibited under
previous Prohibited Lists, as applicable.

The ESA sub-section has been expanded by adding the non-erythropoietic
EPO-Receptor agonists.

In addition, the HIF stabilizer sub-section has been placed in a separate
paragraph to highlight the growing importance of these drugs and to
specifically identify HIF activators such as xenon and argon. Note that

A Corticotrophin releasing factor has now been added as an example. GH
releasing factors have been divided in a more precise categorization and
several examples of each have been added to illustrate their different
pharmacological properties.
 IGF-1 has been moved to the other growth factor section.
 Note that Platelet Derived Plasma preparations are not prohibited.
S4. Hormone and Metabolic Modulators
Trimetazidine was originally included in S6.b based upon the similarity of its
chemical structure to some of the listed Stimulants. It has been moved to the
newly created sub-section S4.5.c as it is pharmacologically classified as a
modulator of cardiac metabolism.
 AMPK activators were re-described to reflect current nomenclature.

S5. Diuretics and Masking Agents
 The title and subsequent paragraphs has been modified by the removal of the
word “other” to reflect that diuretics are not only masking agents but can also
be abused for other purposes such as notably the induction of rapid weight
loss.
 The last paragraph has been reworded for the sake of clarity. The principle and
processes identified in the previous version of the Prohibited List remain
unchanged.
PROHIBITED METHODS
M2. Chemical and Physical Manipulation
 The term “surgical procedures” has been added to the sentence describing the
situations in which the administration of intravenous infusions or injections of
more than 50 mL per 6 hours are permitted when medically required.
SUBSTANCES AND METHODS PROHIBITED IN-COMPETITION
S6. Stimulants:
 For the sake of clarification, topical/ophthalmic imidazole derivatives have been
mentioned as exceptions of non-prohibited stimulants.
 Phenmetrazine, initially in section S6.a, has been moved to section S6.b since
fenbutrazate (section S6.b) can metabolize to phenmetrazine.

 The List now clearly identifies the whole family of phenethylamine derivatives as
being prohibited to address the growing number of illegal, designer stimulants
derived from phenethylamine.
 Trimetazidine has been transferred to the newly created section S4.5.c (see
above).
S9: Glucocorticoids:
 Glucocorticosteroids are described as Glucocorticoids to reflect current
nomenclature and use.
SUBSTANCES PROHIBITED IN PARTICULAR SPORTS
P1: Alcohol:
 Following the request of World Karate Federation (WKF), karate was removed
from the list of sports in which alcohol is prohibited.
P2. Beta-blockers
 The World Underwater Federation (CMAS) has, at its request, been added to
the group of International Federations prohibiting the use of Beta-blockers in
competition for certain disciplines.
MONITORING PROGRAM
 Following sufficient collection of data leading to clear conclusions, monitoring
of pseudoephedrine below 150 microgram per milliliter will cease in 2015.
 Telmisartan, an atypical angiotensin II receptor antagonist with apparent partial
PPARδ receptor agonist properties (prohibited under S4.5.b), was added to the
Monitoring Program to assess the abuse of this substance.
 Meldonium, a drug with potential cardiac effects, was added to the Monitoring
Program to assess the abuse of this substance.
