Reception Staff and Administration Staff Our Reception Staff are here to help you. When telephoning for medical attention our Reception Staff may ask for a few details. The doctors have asked them to make these enquiries so they can help you in the most appropriate way. Our Reception Staff have undertaken special training and always respect patient confidentiality. Practice Nurse(s) The nurses are qualified to advise and treat many minor conditions and it may prove quicker to see a Practice Nurse than wait for a consultation with your doctor, who is always available if you have any cause for concern. When booking an appointment with a Practice Nurse, our Reception Staff may need to ask you the reason for an appointment in order to determine how much time to allocate. Consultations are by appointment. Health Care Assistant(s) The Health Care Assistant(s) are not qualified as nurses but have undergone training to enable them to take bloods, blood pressure, assist with minor surgery, ear syringing, new patient health checks that do not involve medication and do ECGs. All blood tests must be made through either one of the doctors or be a routine monitoring blood test within the Practice protocol. If the service is abused by calling the doctor for requests for repeat prescriptions, appointments or minor illness which could wait until the next surgery session, then other patients could be put at risk. To Register Please bring your medical card to reception; if you have lost your card, we can help you complete a new registration form. Or, please complete the application form on-line, and then come into the surgery to sign. When you come to the Health Centre to register, you will be asked for some form of photo identification (passport or photo-driving licence). You may express a preference to have a particular doctor or to be seen by a specific health care professional for particular services. We will try to comply with any such wishes. We do accept private patients. Appointments Appointments can be made by telephone, in person or via the internet. After the necessary registration procedure you may book, view and cancel on-line a routine telephone appointment with your GP. You can also view and cancel on-line your appointment with any healthcare professional within the surgery. Please ask reception for full details. We also operate a telephone consultation service, whereby you can arrange for a doctor to telephone you to discuss any medical issues. This service also allows patients to be seen faster and more conveniently than under the previous system. A high proportion of problems can be dealt with over the phone. We will be able to offer you a doctor’s appointment within 48 hours or an appointment to see a health care professional within 24 hours. If you wish to see a specific person, you may have to wait longer. Urgent health problems will always be seen the same day. Surgeries are held daily between the hours of 9.00am – 12.00 noon and 2.30pm – 6.00pm, Monday to Friday. Nurse clinics are held daily between the hours of 8.00am and 5.30pm. Early morning doctor surgeries are held on Tuesdays and Fridays at Odiham (from 7.30am) An evening doctor’s surgery is held at Odiham on Wednesdays (6.30 pm to 8.00pm) The Odiham Health Centre is open every Saturday from 08.30 to 12.30 for normal reception services, there is a nurse clinic every week and a doctor surgery once a month. www.odihamhealthcentre.co.uk V.1 2015 3|P age Drs Weaver, Shand & Assadourian – Practice Leaflet (2015) Temporary Residents UK residents can make appointments to be seen as NHS patients. Proof of identity will be required. Overseas patients will be treated as private patients unless they have documentation and proof of identity to confirm entitlement to NHS services. Patients with special needs Odiham and Old Basing Health Centres have suitable access for people with disabilities. If any patient feels that they cannot access any of the Practice service because of their disability, please discuss the matter with the Practice Manager so that we can resolve the difficulty. Dogs All dogs, with the exception of assistance dogs, should be left outside the building. There are metal eyes available to secure a leash if required. Smoking & Food The building is a non-smoking building and we request that this be honoured. We would also request that patients do not eat or drink whilst waiting for their appointments. A cup of water may be obtained from reception on request. Self-Check-in The surgery has an automated self-check-in touch screen. This is a simple to use system that enables the patient to check themselves into our appointments system. Should you feel uncomfortable about using such a system, you can obtain help, or book in with our Reception Staff in the traditional manner. Access to Patient Records The Practice charges the following fees where a patient requests to access their own medical record: Viewing Records Only – Free if the records have been updated within the last 40 days. Otherwise, £10. Obtaining Copies of Health Records If held on computer – maximum £10 If held in another media – maximum £50 If held on a combination of computer and other media – maximum £50 Car Parking A car park is provided for patients who are visiting the surgery, with designated parking spaces for the use of disabled patients. All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. This chaperone may be a family member or friend. On occasions you may prefer a formal chaperone to be present. Your healthcare professional may also require a chaperone to be present for certain consultations in accordance with our chaperone policy. If you wish to have a member of the Practice staff present during you consultation please mention this to our Reception Staff when booking your appointment, or to the doctor at your consultation, and it will be arranged. Mobile Phones We allow mobile phones to be used within the surgery building, but please ensure you turn them off before going into the doctor’s consulting room. Zero Tolerance We will treat our Patients with respect, courtesy and will not discriminate against them in any way on the grounds of age, sex, colour, race, nationality, ethnic or national origin or disability, sexual orientation, religion or religious or philosophical belief. Physical violence and verbal abuse is a growing concern. GPs, Practice Nurses and other Practice staff have the right to care for others without fear of being attacked or abused. We ask that you treat your GP and Practice staff properly – without violence or abuse. We strongly support the NHS policy on zero tolerance. Anyone either phoning or attending the Practice who abuses any staff member or patient, be it verbally, physically or in any threatening manner whatsoever, will risk removal from the Practice list and be reallocated with the assistance of the North Hampshire Clinical Commissioning Group. In extreme cases we may summon the Police to remove offenders from the Practice premises. Home visits As home visits are for those patients too ill to attend surgery, please only request a home visit if you (or the person you are calling on behalf of) is too ill or frail to attend the Practice. Please remember the doctor can see four patients in the time it takes to make one house call. If you are requesting a home visit, please try to telephone the surgery before 11.00am. Please give clear directions so the doctor can locate your house. If you have an outside light, please ensure that it is switched on during the hours of darkness. Also, if you have a dog that is not familiar with visitors, please ensure that it is securely locked away. Prescription Requests Patients who need to take regular medication will be issued with a computerised list of their authorised repeat medicines. If you require a prescription, either o Tick the item(s) on your list and return it to the surgery. o Telephone the surgery – after 11.00am if possible. o If you want the prescription posted to you, please provide a stamped self-addressed envelope. o Use the Internet Prescription Request system – you will need to register for this facility. Please enquire at Reception or see our website for full details (www.odihamhealthcentre.co.uk). Your prescription will be ready for collection within 2 working days. When appropriate, we will ask you to make an appointment with your doctor to review your treatment. We regularly review your medication and will initiate changes when considered to be of benefit. Please make sure you only request those items that are needed. Sickness/Fitness Certificates Now known as Statement of Fitness for Work or “Fit Note” You should complete a self-certification form (SC2, available from reception, from your employer or the HMRC website (www.hmrc.gov.uk)) for any illness lasting less than seven days. We can only provide NHS certificates for illnesses causing absence from work for 7 days or more. www.odihamhealthcentre.co.uk V.1 2015 6|P age Drs Weaver, Shand & Assadourian – Practice Leaflet (2015) If an employer requests a certificate for an illness lasting less than seven days, a private certificate can be issued, for which a fee is payable. NHS sickness certificates are only be used in connection with sickness absence from work. Sickness certification for all other purposes will require a private certificate of statement. General Services The doctors and Practice nurses provide the full range of medical services normally expected at a GP surgery. We are a teaching Practice and there will often be fully qualified doctors and nurses working at the Practice who are learning more advanced medical techniques and management. Annual Medical Review For patients with chronic medical conditions such as asthma, COPD, epilepsy, severe mental ill health, cerebrovascular disease, learning disabilities and chronic kidney disease. Six-monthly Medical Review For patients with Coronary Heart Disease, Diabetes and Hypertension. Medication Review Your medication is under constant review. We do like to discuss your regular medication with you at least every six months. The review can often be done by telephone. The medication review date on your prescription is for your doctor to review and decide whether a telephone of face-to-face review is required. The Practice Nurses Offer a wide range of services including new patient, well person, diabetes, asthma, COPD and heart checks, health education, health promotion, child immunisation, travel and other vaccinations, flu vaccinations (starting in October each year), cervical smears, blood tests, dressings, treatment of minor injuries, ear syringing, hearing tests, blood pressure measurements, dressings and removal of stitches. Referrals to Hospital Patients have the right to be referred to the Hospital of their choice. There is a system called ‘Choose & Book’, which can allow you to choose the hospital and your appointment time. Carers We would like to know if you are caring for someone who has long-term health problems. We believe that Carers need support as well. Forms are available in the surgery and can be printed off on-line. Clinics and one-to-one help are provided. Additional clinics are held locally; for further information please ring 0845 602 4663. Test Results Results of blood tests, X-Rays etc. can only be given to the patient themselves for reasons of confidentiality. Parents/guardians can obtain the results for children under 16 years, unless the child requests otherwise. Blood test results are usually available within 5 days, x-ray reports within 7 days and hospital letters within 4 weeks. Immunisations We can provide the full range of child, occupational and travel vaccinations. Some vaccinations are covered by the NHS, but for others a fee is payable. For travel immunisation, please make an appointment with the Practice Nurse at least 8 weeks before your intended departure. We are a registered yellow fever centre. Currently there is a national shortage of yellow fever and typhoid vaccines. Please discuss your requirements with a Practice Nurse at least 8 weeks before departure as we cannot guarantee their availability. Child Health Child development checks are organised by the Health Visitors, who will advise when the checks are due. You will receive a request from Child Health to book an appointment, please contact the surgery to arrange it so that your child does not miss this important protection. Odiham Cottage Hospital, Rosefield Day Centre Mondays 09.30 – 11.00 Maternity Services Pre-conception advice is offered. We offer a full range of maternity services with the help of our community midwife. Please make an antenatal booking appointment with the midwife at 6-8 weeks of pregnancy. Antenatal clinics and classes are held regularly. Postnatal care is shared between the midwives, health visitors and doctors. Please make a 30 minute appointment with your doctor for the 6-week postnatal check for you and your baby. Minor Surgery Services We offer a variety of minor surgical procedures, including removal of small skin lumps and joint injections. Contraceptive Services All the doctors provide a confidential family planning service. You can ask to see one of the female doctors if you prefer. For coil (IUCD) insertions and contraceptive implants you must discuss with Dr Shand or Dr Pizzotti first. If you need the “morning after” pill please request an urgent telephone appointment. Cervical Smears The national recall system is for all women aged between 25 and 65 years: you will be sent a reminder when your smear is due. This is currently every 3 years from age 25 to 50, and every 5 years from 50 to 65. Please make an appointment with the Practice Nurse at the surgery on receipt of your reminder letter. The Winchester Laboratory informs each patient of the result of their smear test and will inform the patient when the next smear is due or if any other action needs to be taken. www.odihamhealthcentre.co.uk V.1 2015 8|P age Drs Weaver, Shand & Assadourian – Practice Leaflet (2015) Health Visitors Health Visitors are qualified nurses and have additional training in child health and development, will monitor your child’s growth, answer any child health queries and help mothers to cope with children under five, advising on things like hygiene, safety, feeding and sleeping. They also co-ordinate child immunisation programmes and organise special clinics or drop-in centres. Health Visitors can also provide you with help and information regarding such issues as emotional problems, relationship difficulties, family planning and health problems and advise on healthy eating, keeping warm and getting the right exercise. Midwife Midwives care for and support pregnant women, their partners and new babies before, during and after the birth. They monitor the health of the mother, counsel her on health issues and explain the options for delivery of the baby. Their job also involves reassuring parents, running antenatal and parenting classes, taking care of the mother and baby during labour and birth and giving advice on breast feeding. Community Nurses Community Nurses assess, plan and manage the care of sick and disabled patients of all ages in the patient’s own home, general practices and residential nursing homes and also provide support for their Carers. Individual care is planned, and advice on further help of services is given including health education. Anyone at home can self-refer to this service. They have a close liaison with the Hospital, GPs, the local hospice and other specialised services i.e., Macmillan Nurses, Continence Advisors, Diabetic Nurse Specialists, Community Paediatric Nurses and others. The community nurses administer drugs, give injections, dress wounds, take blood samples and give personal care.
