By early 1946, Dean Roy Kracke had begun a search for a chief of anesthesia for Jefferson-Hillman Hospital where the medical students trained. Dr. Kracke first offered the position to Dr. John Adriani of the legendary Charity Hospital in New Orleans. When Dr. Adriani turned down the offer, Dean Kracke then offered the post to Dr. Alice McNeal, an anesthesiologist for more than twenty years at Presbyterian Hospital in Chicago. A 1921 graduate of Rush Medical College, Dr. McNeal's specialty training included a year of study under Dr. Ralph Tovell at Hartford Hospital in Connecticut. On April 3, 1946, Dr. McNeal wrote her acceptance letter to Dean Kracke for the position of Chief, Division of Anesthesiology, Department of Surgery. By the spring of 1948 Dr. McNeal, unhappy with conditions at the medical school in Birmingham, was considering a lucrative offer to return to Chicago. In order to keep her, Dean Kracke proposed to create an autonomous anesthesia department at the hospital, make Dr. McNeal Professor and Chair of that department, and raise her salary to $8,000 per year [several thousand less than the Chicago offer]. Dr. McNeal accepted Dean Kracke's offer and the Department of Anesthesiology, one of fewer than twenty such autonomous academic departments in the United States at that time, were officially created on August 1, 1948.
