scholarship(s), we would like to recognize you on Tuesday, May 17, 6:00 p.m. at our annual Senior Awards
Ceremony (This is the same event that we acknowledge our seniors who are receiving academic department and, GPA awards, etc.)
Here is the information we need from you:
1.
Your name, phone number and student ID number at the top of this page
2.
The name of the University, College, Trade School or Military Branch you will be attending.
3.
In the boxes on the back of this page, list all scholarships that you have been awarded. Please also note which one(s) you will be accepting and which ones you will not.
4.
Please be sure to attach a copy of your award letter if you will be attending an out-of-state school and or receiving a community foundation award. (See Ms. Phillips if you have any questions)
If you have received an athletic scholarship we want you to share that information too, as Principal Serrano will be acknowledging athletic scholarships during our graduation ceremony. (Please attach a copy of your awards letter and return it a Blue Form. See Ms. Phillips for the form.)
* Please do not include monies that you will be receiving through Federal/State Grants. For example, the ASU
Obama Scholar and the University of Arizona Assurance Programs are Grant Programs. Grant programs are based on financial need and not awarded based on Academic Merit. Grant awards will not be acknowledged during the Senior Awards night.)
**If you are receiving a Military Academy or ROTC scholarship, or if you are enlisting in the military, please contact Ms Phillips ASAP.
