EPA Work Sheet* 1. EPA Title Improve care for a population of patients 2. Description of the activity The 21st century physician needs to understand population health in order to optimize care. Populations can be defined by sociodemographics, disease states, and/or active practice patients to name a few. Caring for populations requires a maturation in professionalization from a focus on the physician-patient dyad to a recognition of responsibility to populations, communities and society at large. This activity requires that the physician utilizes technology (e.g. databases) to seek and analyze data on populations, develops improvement plans based on that analysis, and documents the outcomes. The ability to develop improvement plans requires competence in risk/benefit and cost/benefit analysis. __Patient Care __Medical knowledge _X_Practice-based Learning and Improvement __ Interpersonal & Communication Skills _X_Professionalism _X_Systems-based Practice _X_Personal and Professional Development See attached mapping document 3.
