Abstract Call for Speakers
WANTED: TWO DPCPs TO SHARE SUCCESS STORIES!!
The Learning Event
TWO virtual learning events, called ePACE, are planned for August 23rd and September 11, 2012. We are
looking for two DPCPs to share their success stories at each of the virtual events. The focus of the ePACE
learning is: CORE INTERVENTION 2: SUSTAINABLE DIABETES SELF-MANAGEMENT EDUCATION.
Emory University’s Diabetes Training and Technical Assistance Center (DTTAC) is pleased to host this event, in
collaboration with the Centers for Disease Control and Prevention’s Division of Diabetes Translation (DDT) and
the National Association of Chronic Disease Directors’ Diabetes Council (NACDD).
The Success Story
Interested DPCPs are asked to submit an abstract demonstrating proven success in Core Intervention Area 2:
Sustainable Diabetes Self-Management Education using at least one of two specific strategies:
 2.1 A: Expanding access to DSME/T programs that meet national standards and demonstrate improved
behavioral and/or clinical outcomes for people with diabetes (e.g., ADA recognized or AADE accredited
DSME/T programs)
 2.2 Advocating for the implementation of policies that promote financial sustainability/reimbursement for
DSME/T programs (e.g., Medicaid reimbursement for DSME/T programs, bundled reimbursement)
The Audience
 The August 23rd session will be delivered for CDC Project Officers and Evaluators only. This will serve as
a pilot to test the approach and flow.
 The September 11th session will be open to both DPCPs and DDT Project Officers and Evaluators.
The Abstract Submission
 Complete the attached Submission Form: Submission Deadline is Friday, July 6, 2012.
 Submit electronically to Rosa Laboy-Hernandez at rlaboy@emory.edu.
 We will notify speakers in mid-July.
The Requirements
• Oral presentations will be 15 minutes in length with 10 minutes allotted for Q&A.
• Handouts, support documents, or other resources must be provided by July 26, 2012.
The Presenter’s Delivery Options
This is a virtual conference using a web based application that can be accessed from any computer with an
internet connection. Confirmed presenters may request to deliver the presentation from Atlanta where
technical requirements will be provided by DTTAC; travel expenses may also be covered.

On the next page, please provide a brief abstract (limit to two pages) describing your “success story”

Purpose/objective of strategy, model, or approach to address challenge(s)
Outcomes of “success” measured
Primary audience(s) and reach
Role of the DPCP and partners
Translatability to other states, partners, and/or other chronic disease programs
Capacity required to replicate (to include resources required, both fiscal and human)
