NOHS PTO Scholarship Application Deadline: Thursday, May 1, 2014 at 4:00pm The NOHS PTO will honor two graduating students from the class of 2013 (one female and one male) by awarding each a $500 cash scholarship. These grants are intended to recognize and reward students who have demonstrated a history of participation in school activities, community service and/or employments during high school, contributing to the betterment of an organization or the NOHS community, and who are good students of sound character. The Selection Panel will utilize the following criteria when making their selection: Involvement in School Activities 20% Community Service and/or Work Experience 20% Contribution, during your high school years, to the betterment of an organization or the NOHS community (short answer) 20% Academics 20% Character (1 letter of recommendation) 20% ************************************************************************************************** Application process 1. Complete the application cover sheet. It is available in the NOHS Guidance Office, may be downloaded from the NOHS Guidance Office blog, or may be replicated by any means. Answer the application questions and attach to the cover sheet. Additional materials (including but not limited to resumes, additional letters of recommendation beyond the one requested, portfolios, etc) will not be distributed to the Selection Panel nor considered. 2. Please use your name or other identifiers only on the cover sheet. Prior to consideration by the Selection Panel, committee members will review applications to ensure anonymity. Note: no person who has a student currently enrolled at NOHS or MBMS will take part in the selection process. 3. Request a letter of recommendation (please note only 1 letter will be considered) from a teacher, administrator, counselor, employer, or community leader that speaks to your character. This should be submitted in a sealed envelope, with the author’s signature over the seal, and a piece of tape placed over the signature. The applicant must submit the letter with their application. 4. Return the completed application, including the letter of recommendation, to the NOHS Guidance Office by Thursday, May 1, 2014 at 4:00pm. 5. We are not looking for essay answers; please respond using a list format. Ping Pong Club 11th Member: participated in 10 tournaments, winning 2 12th Equipment Manager: responsible for set up and maintenance of equipment for practice and tournaments. 2. Community Service and/or Work Experience (20% or a maximum of 5 points) In what community service projects (work with not-for-profit organizations) have you been involved? For how many years have you been involved with these projects? In what way were you involved (member, officer) with these projects? Note: Community service work may include work done through groups at NOHS ex: NOHS Beta Club. We are not looking for essay answers; please respond using a list format. Save the Elephant Club 9-11th Member: the club raises awareness of the decreasing habitat of the African Elephant. I passed out flyers once a month during the school year inviting the public to our presentations. 2b. Have you been employed while attending NOHS? If so, for how many years and in what positions? Sally’s Ice Cream Shop 12th Counterperson. Worked 5 hours a week serving customers. 3. Contribution, during your high school years, to the betterment of an organization or the NOHS community (20% or a maximum of 5 points) Every effective organization has both leaders and followers. Using a personal experience while a NOHS student as an example, write a short answer (maximum 250 words) where you explain the role you took, and how in that position you bettered the organization or the NOHS community as a whole. 4. Academics (20% or a maximum of 5 points) By signature on your application you agree to allow the NOHS Guidance Office to provide the Scholarship Committee your weighted GPA (as calculated at the end of the fall semester 2013). Points will be awards based on the following: 5 points are awarded for GPAs of 98%+ 4 points are awarded for GPAs of 93-97.99% 3 points are awarded for GPAs of 88-92.99% 2 points are awarded for GPAs of 83-87.99% 1 point is awarded for GPAs of 78-82.99% 5. Character (20% or a maximum of 5 points) Please provide a letter of recommendation from a teacher, administrator, counselor, employer, or community leader that speaks to your character. The letter should be included with the application when you submit it to the counselors’ office.
