11/2013- Present: Jefferson Medical College/Christiana Care Health
System. William Weintraub, M.D.
We developed models to predict 30-day inpatient readmission to our institution
for a cohort of patients who were revascularized at our institution between January
2010 and December 2013. Our model would allow providers and health systems to
target high-risk patients with enhanced discharge planning during the course of
the hospitalization, and this may improve the ability to avoid readmissions.

11/2013- Present: Jefferson Medical College/Christiana Care Health
System. William Weintraub, M.D.
We developed a model where a care management team that includes social
workers, nurses, a pharmacist, a health ambassador, a care management supervisor
and a physician adviser works with more than 2,200 patients with ischemic heart
disease to make the transition from hospital to home a safe, efficient and
successful. The team explains medication, schedules future doctor appointments
and makes sure other services are in place. At home, daily readings of weight,
blood pressure and oxygen saturation may be collected. The readings alert Care
Link Services to potential problems. Ongoing data collection helps the care team
to adjust treatments as needed. This project is providing an opportunity to partner
with our patients to improve their health. The program is showing potential to
improve our patient’s health status, and reduce emergency room visits and
hospitalizations. This in turn drives down costs and improves value.

05/2013- Present: Jefferson Medical College/Christiana Care Health
System. Roger Kerzner, M.D.
We are developed a multidisciplinary team model in Cardiac Care Intensive where
a cardiologist, a medical intensivist, a cardiology fellow, internal medicine
residents and interns, ICU nurses, ICU pharmacist and respiratory therapists
works together in the team to coordinate the complex care of patients in the
Cardiac Care Intensive Unit. This approach in expected to impact many outcomes
including patients’ mortality, morbidity and length of stay in both ICU and the
hospital.

10/2014- Present: Jefferson Medical College/Christiana Care Health
System. Andrew Doorey, M.D.
We assessed the impact of the preset cath lab computer system algorithm on the
hemodynamic assessment of cardiac output and aortic valve area. We found that
whichever CO calculation was done first by the CCL computer system became the
determinative output, appearing on the final report and being used to calculate the
official AVA. That had an intense clinical outcome as the recommendation for
surgery (AVA <1.0 cm2) was different between the CO methods in 12 cases (12% of

We evaluated the safety of using pressure wire for aortic stenosis assessment.
Pressure wire may offer a safe and higher quality technique to assess the validity of
echo results in assessing the severity of aortic stenosis.
We found that cath Assessment of the aortic valve area (AVA) was discordant with
echo by more than 0.2cm2 in 48%, 0.3cm2 in 25% and 0.5cm2 in 7% of patients.
Values changed to over or under the surgical threshold of AVA <1cm2 in 30% of
the patients (Pearson correlation of 0.47). Mean echo gradients had better
correlation with cath gradients (75%, Pearson correlation of 0.82). Urgent surgical
valve replacement was avoided in 18 patients (24%). No clinical strokes or TIA
were observed in the 30 days after procedure.


on the patency of radial artery and future impact that may have on using that

We were trying to investigate the Safety of Beta Blockers in the Acute
Management of Cocaine-associated Acute Coronary Syndrome.
We found that "In patients with cocaine-associated ACS Beta Blockers did not
appear to change the incidence of the primary composite outcome of death, MI,
cardiac arrhythmia or any of the secondary outcomes of individual rates of death,
MI or cardiac arrhythmia”.


We were trying to study the effects of Transvenous pacemaker and implantable
Cardioverter defibrillator on mitral (MR) and tricuspid (MR) regurgitation
We found that “The implantation of Transvenous PPM or ICD worsens TR
probably through direct mechanical interference. Increased MR severity was seen
only in patients with high RV pacing percentage, which is likely the consequence
of Interventricular dyssynchrony induced by RV pacing. "


We were trying to prove the significance of Transesophageal Echocardiography use
in the prediction of the occurrence of Aortic Dissection depending on the
measurements of Aorta wall thickness in patients with significant risk factors in
addition to the Aorta diameter, which is being used.
We found that "The aortic wall thickness and intimal/medial thickness detected by
TEE at the level of 40cm beyond incisors is greater in patients with aortic
dissection compared with PFO patients independent of aortic diameter. This may
be useful in identifying patients with an increase risk of developing aortic
dissection".


We were trying to investigate the Safety of Beta Blockers in the Acute
Management of Cocaine-associated Chest Pain.
We found that "In patients with cocaine-associated chest pain Beta Blockers did
not appear to change the incidence of troponin rise. Beta 1-Selective Beta Blockers
did not appear to worsen troponin levels compared to mixed Beta 1/Alpha adrenergic antagonists. There were no cases of unopposed alpha activity reported”.

12/2013- 02/2014: ACC Delaware Chapter FIT Leader. ACC.
Works with the ACC leadership to enhance fellows’ involvement in the ACC
communities and activities.

3/2014: ACC Patient Navigator Program working group, ACC
The ACC developed the ACC Patient Navigator Program to address these issues
through a personalized approach focused on meeting the needs of heart disease
patients before and after they leave the hospital. The program will support a team
of caregivers dedicated to helping patients overcome challenges during their
hospital stay and in the weeks following discharge when they are most vulnerable.
The ultimate goal of the program is to develop and support a culture of patientcentered care that can be implemented in other hospitals.

07/2013- Present: ACC FIT Quality Improvement working group, ACC
The ACC has a number of quality initiatives, such as Hospital to Home and Door
to Balloon. ACC Science and Quality staff would like to better engage Fellows in
Training and get their help implementing these initiatives at their training
programs.

07/2013- 07/2014: ACC FIT CardioSource Communities Working Group
The purpose of this working group is to manage and sometimes develop for the
biomarkers and sleep apnea communities as well as for CardioSource hot topics
and cases for CardioSource.

08/2014- Present: Christiana Care Quality and Safety Council:
I am representing the cardiology section in this collaborative council among all
section and department to coordinate effort over all inpatient and outpatient
effort to improve the quality of care and guarantee the safety of our patients.

