Case Scenario for UCF Live Standardized Patients Emergency Providers and Nurses Patient A CC: Acute Pelvic Pain VS: T 98.2, HR 90, BP 124/80, RR 20, 98% on room air Pain 4/10 Labs: Point-of-care urine pregnancy test is negative ESI: Level 3 Case Scenario for UCF Live Standardized Patients Emergency Providers and Nurses Patient B: Pre-menopausal CC: Heavy and painful periods x 2 months VS: T 98.6, HR 76, BP 116/70, RR 16, 98% on room air Pain 4/10 ESI: Level 3 HPI: Diffuse, cramping pelvic pain during menses for the past 2 months. Pain had been 4/10 but increased to 6/10 this month. Menses every 28 days lasting 8 days. No change in heavy flow for the first 2 days, but now passed clots which is new prompting her to come to the ED. Wears a tampon and pad the first few days changing every 3-4 hours. Often has to call off work for the first two days of her menses. Has tried many over-thecounter meds including ibuprofen and acetaminophen, but nothing relieves the pain. Warm compresses to her lower abdomen help take the edge off. ROS: Denies lightheadedness, dizziness, chest pain, palpitations, shortness of breath, vaginal discharge, urinary symptoms. PMH: PTSD Iron deficiency anemia PSH: Appendectomy 2007 OB/GYN: G5P4Ab1 LMP 8/8/14, as above Not sexually active for 6 months Nexplanon® since 10/2013 History of chlamydia in teens, denies history of pelvic inflammatory disease SocHx: Divorced, lives alone Non-smoker, no alcohol or drug use Denies IPV Allergies: PCN Medications: Ferrous gluconate daily, Nexplanon® Labs: Point-of-care urine pregnancy test is negative Case Scenario for UCF Live Standardized Patients Emergency Providers and Nurses Patient B: Post-menopausal CC: Vaginal bleeding x 2 days VS: T 98.6, HR 76, BP 116/70, RR 16, 98% on room air Pain 4/10 ESI: Level 3 HPI: Light to moderate vaginal bleeding started acutely 2 days ago without inciting event. Denies abdominal/pelvic pain. No clots passed. LMP was 3yrs ago and hasn’t had any bleeding until now. Does not use anything in vagina. Never on HRT. ROS: Denies lightheadedness, dizziness, chest pain, palpitations, shortness of breath, vaginal discharge, urinary symptoms, and rectal bleeding. PMH: PTSD PSH: Appendectomy 2007 OB/GYN: G0 LMP 3 yrs ago Not sexually active for 6 months Remote history of chlamydia Denies history of pelvic inflammatory disease SocHx: Divorced, lives alone Non-smoker, no alcohol or drug use Denies IPV Allergies: PCN Medications: Calcium/Vitamin D
